不同保存温度对血浆部分凝血因子活性的影响 被引量:25

摘要 目的:探讨在不同保存温度,不同保存时间血浆部分凝血因子活性的变化,为临床选择不同血浆输注提供实验室依据。方法:采集10袋(200mL/袋)的ACD-B血液保存液保存的血液,在采血后1h内留样10mL作为对照组样本;每份全血在无菌条件下平均分为2袋,作为2个实验组,热合封口。第1组保存于(4±2)℃,保存48h;第二组先保存于(22±2)℃,保存6h之后,转移到(4±2)℃保存42h。两个实验组分别在采血后6h,24h和48h留取样本。将对照组及实验组样本在取样后立即离心,分离血浆,于-18℃保存30d后,进行凝血因子Ⅴ(FⅤ)、凝血因子Ⅷ(FⅧ)、纤维蛋白原和凝血因子Ⅶ的检测。结果:两组血浆在不同温度、不同保存期的凝血因子Ⅴ、纤维蛋白原和凝血因子Ⅶ水平无显著改变;而在保存8h后,实验组血浆凝血因子Ⅷ水平均低于对照组凝血因子Ⅷ水平(P<0.05)。在保存24h后,实验组血浆凝血因子Ⅷ水平进一步下降(P<0.05)。结论:在采血后24h保存期内,血浆中凝血因子,除凝血因子Ⅷ外,凝血因子Ⅴ,纤维蛋白原和凝血因子Ⅶ活性均没有明显改变;凝血因子Ⅷ水平在4℃保存8h后下降14%;在保存24h后,凝血因子Ⅷ水平下降约30%;在保存48h后,凝血因子Ⅷ水平下降约40%。实验结果表明,在新鲜冰冻血浆供应紧张的情况下,除非需要大剂量补充凝血因子Ⅷ(病人凝血因子Ⅷ水? Objective To observe effect of whole blood stored at different temperature and time elapse on some plasma clotting factors.Methods A 200 mL unit of whole blood was collected from 10 volunteer donors.A whole blood sample was drawn from each unit and a plasma sample was separated by centrifugation within 1 hour after collection as the control group.Each 200 mL unit was divided into 2 half units as 2 groups.One group was stored at 4℃for 48 hours and the other group was stored at 22℃for the first 6 hours and then 4℃for the following after collection.At the time point of 8 h,24 h and 48 h after collection respectively,plasma samples were drawn from each group of the whole blood units.All plasma samples were separated by centrifugation immediately after the samples' drawing and frozen at 18℃for 30 days.Factors Ⅴ,Ⅷ,fibrinogen and factor Ⅶwere measured by ACL2000 after thawing all the samples.Results No significant changes were observed in factors Ⅴ,fibrinogen and factor Ⅶ. Factors Ⅷin both groups was significantly reduced compared with the control group.Conclusion The coagulation factor activity for all factors measured showed no significant change over 24 h and 48 h storage period with the exception of factor Ⅷrespectively.Fresh frozen plasma and frozen plasma in 24 h after collection could be used to complement factors Ⅴ,fibrinogen,factor Ⅶwith exception of factors Ⅷ.
