摘要
目的探讨细胞增殖与细胞凋亡在贲门癌发生发展中的作用。方法应用免疫组化法检测PC N A、p53、Bcl-2在30例贲门癌、10例胃溃疡胃壁组织中的表达和分布,采用病理图像分析的方法进行灰度检测,并对胃壁上皮组织或癌组织中凋亡细胞进行计数。结果观察和比较自胃远端切缘、癌旁至癌灶等胃壁组织中的变化,发现PC N A呈显著的增高趋势,癌灶组织中PC N A呈强阳性表达;p53在正常胃黏膜组织中不表达,在癌灶组织中呈强阳性表达;Bcl-2在癌旁组织中呈强阳性表达,显著高于癌灶组织;细胞凋亡指数在癌灶组织中显著降低。胃溃疡病灶边缘的胃黏膜组织PC N A呈阳性表达,p53呈阴性表达,Bc1-2呈弱阳性表达,细胞凋亡指数显著增高。结论在非典型增生的贲门黏膜组织中,细胞增殖和细胞凋亡均非常活跃,而一旦细胞凋亡受到严重抑制,即可导致贲门癌的发生。
Objective To investigate the role of cellproliferation and apoptosis on the carcinogenesis and development in cardiac carcinoma. Methods With immunohistochemistry stain,it was measured the distribution and expression of PCNA,p53 and Bcl-2 in the gastric tissues of the cardiac carcinoma(n=30), gastric ulcer(n=10) and measured their gray scales with the method of the pathomorphological analysis. The number of apoptosis cell was accounted in the gastric epithelial tissue or cardiac carcinoma tissue. Results The expressive level of PCNA gradually increased in the tissues from the gastric far-end cutting edge, peripheral to center areas of cardiac carcinoma and the expression was fortis positive in the carcinoma tissue;The expression of p53 was negative in the normal gastric mucous membrane and fortis positive in the carcinoma tissue; the expression of Bcl-2 was fortis positive in the peripheral area of the carcinoma tissue and it was noticeable higher than that in the carcinoma tissue. Apoptosis index (AI) was noticeably decreased in the carcinoma tissue. In the gastric mucous membrane of the gastric ulcer focus edge, the expression of PCNA and p53was positive; the expression of Bcl-2 was low positive; AI noticeably increased. Conclusion Both of the cellproliferation and apoptosis were very active in the atypical hyperplasis cardiac mucous membrane. If the mechanism of the apoptosis was highly restrained, the carcinogenesis of cardiac carcinoma was induced in the cardiac mucous membrane.
