摘要
目的:量化研究儿童上呼吸道阻塞治疗后行为学改善的意义。方法:选择2001-05/2004-06在解放军第463医院和沈阳医学院第二附属医院耳鼻喉科门诊就诊的患儿180例为研究对象,男108例,女72例。对患儿行扁桃体、增殖体手术,分别于术前1天和术后6个月采用儿童上呼吸道阻塞症状调查表及儿童行为调查量表(CBCL)对其家长分别进行问卷调查并评分。结果:上呼吸道阻塞患儿术前至少有2项以上阳性症状,睡觉打鼾、张口呼吸及休息不好是最常见症状,睡眠呼吸暂停仅占28.9%;部分患儿在活动情况、社会情况、学习情况及行为问题犤X,95%CI:5.28(4.68~5.85),5.20(4.72~5.65),6.68(6.28~7.08),29.32(22.85~36.23)犦等方面存在异常。经手术治疗后,除遗尿外,患儿各项症状及活动情况、社会情况、学习情况、行为问题均明显优于术前(P<0.01);评分改变情况与家长主观反映符合率100%。结论:儿童上呼吸道阻塞可以对儿童造成各种危害。对于上呼吸道阻塞儿童,应该积极的进行手术治疗,不但可以使上呼吸道阻塞症状减轻或消失,而且可以使由此造成的行为学方面的问题得到改善。
AIM:To quantitatively study the behavioral improvement in children with upper airway obstruction after surgery. METHODS:Totally 180 children(108 boys and 72 girls) with upper airway obstruction were selected from the Department of Otorhinolaryngology,the 463 Hospital of Chinese PLA and Second Affiliated Hospital,Shenyang Medical College from May 2001 to June 2004.All the cases underwent adenotonsillectomy,and their parents were investigated by using the scale of upper airway obstructive symptoms in children and child behavior checklist(CBCL) 1 day before and 6 months after surgery. RESULTS:Before surgery, there were at least two positive symptoms in children with upper airway obstruction,and snoring,mouth breathing and no good resting were the most common symptoms,only 28.9% of the children had sleep apnea,and part of the children had abnormalities in activity,social,learning and behavioral status[X,95% confidential interval(CI):5.28(4.68 to 5.85),5.20(4.72 to 5.65),6.68(6.28 to 7.08),29.32(22.85 to 36.23)].After surgery, all the clinical symptoms, except for enuresis, were improved significantly(P< 0.01),and the coincident rate of the scores with the subjective reflection of the parents was 100% . CONCLUSION:Upper airway obstruction in children can cause various harms to them. Active adenotonsillectomy to children with upper airway obstruction cannot only relieve or resolve their clinical symptoms,but also improve their behavioral problems caused by the disease.
