Isola钉棒系统三维矫形改善脊柱侧凸患者冠、矢状面及旋转畸形的改善效果 被引量:1

Three-dimensional correction with Isola pedicle screw-rod system for the impro vement of coronal,sagittal plane and rotational deformity in scoliosis patients

摘要 目的:评价应用Isola钉棒系统矫正脊柱侧凸冠状面、矢状面和轴状面畸形患者的术后功能效果。方法:回顾性分析西安市红十字会医院脊柱科2001-03/2003-06应用Isola钉棒系统治疗脊柱侧凸患者22例。特发性侧凸18例,先天性侧凸4例。男13例,女9例;年龄12~18岁,平均14.5岁;术前冠状面Cobb角45°~83°,平均62°,矢状面Cobb角12°~54°,平均34°,椎体旋转按Nash-Moe分型I度6例,II度13例,III度3例。结果:术后冠状位矫正为20°~31°,矫正率为62%~85%,平均69%,矢状位矫正为21°~32°,平均26°。椎体旋转矫正I度。随访9个月~3年,平均2.4年,植骨融合良好,矫正角度丢失率<4.4%。结论:应用Isola钉棒内固定系统校正脊柱侧凸,利用悬臂技术产生的平移力获得冠状面良好矫正率的同时,可获得矢状面的矫正,并保持躯干平衡。 AIM:To evaluate the postoperative functional effects of Isola pedicle screw r ob system for correcting the coronal,sagittal and axial plane deformity of scoli osis patients. METHODS:Retrospective analysis was performed in 22 patients with scoliosis tre ated with Isola pedicle screw rob system in the Department of Spine Surgery of Xi'an Red Cross Hospital from March 2001 to June 2003.Eighteen cases were idiopa thic scoliosis and 4 were congenital.There were 13 males and 9 females aged 12 t o 18 years with a mean age of 14.5 years old.The preoperative Cobb's angle in co ronal plane showed an average of 62°(ranging from 45°to 83°) and 34°(ranging from 12°to 54°) in sagittal plane.Vertebral rotation ranged from Ⅰto Ⅲ,Ⅰin 6 cases,Ⅱin 13 cases and Ⅲin 3 cases according to Nash Moe typing. RESULTS:The postoperative coronal angle was corrected from 20°to 31°at the m ean correction rate of 69%(62%to 85%) in coronal position,and from 21°to 32 °with the mean angle of 26°in the sagittal position.Thoracic kyphosis in posto peration was measured an average of 26°(21°to 32°).Rotation of the vertebra r otation was improved I degree.All the patients were followed up for 9 months to 3 yeans,average 2.4 years.All cases obtained well fused implanted bone.The corr ection angle was lost less than 4.4 percent. CONCLUSION:It can get satisfied correction rate in the treatment of scoliosis by using Isola pedicle screw rob system.Under the shifting force produced by ut ilizing rib technology,not only a good correction rate was gotten in coponal pla ne,but also the sagittal deformity was corrected and kept in a good trunk balanc e.
