肾移植术后高脂血症患者碱性成纤维细胞生长因子和受体mRNA表达及辛伐他汀降脂治疗对其影响

摘要 目的 研究肾脏移植术后高脂血症患者外周血单个核细胞中碱性成纤维细胞生长因子(bFGF)及其受体FGFR2mRNA表达水平及降脂治疗后的改变。方法 肾移植术血脂正常组、血脂增高组及正常对照组各30例,血脂增高组应用辛伐他汀20mg/d进行降脂治疗,于服药后1. 5、3个月复查血脂,同时应用逆转录-多聚合酶链反应方法检测外周血单个核细胞碱性成纤维细胞生长因子及其受体mRNA表达。结果 伴高脂血症的肾移植受者辛伐他汀降脂治疗1. 5个月血脂水及外周血单个核细胞中bFGF及其受体FGFR2mRNA显著高于血脂正常组及对照组,辛伐他汀治疗1. 5个月后血脂水平及bFGF、FGFR2mRNA表达水平显著下降,治疗3个月时进一步下降。结论 伴高脂血症的肾移植受者bFGF及其受体FGFR2mRNA表达水平上调,可能是高脂血症引起动脉硬化、慢性移植肾病的机制之一。辛伐他汀降脂治疗可使碱性成纤维细胞生长因子及其受体的表达下调。 Objective To investigate the mRNA expressions of basic fibroblast growth factor(bFGF) and its receptor FGFR2 in peripheral blood mononuclear cells(PBMC) of renal transplant recipients with hyperlipidemia and the effect of simvastatin on them.Methods Sixty recipients were selected from 167 renal transplant recipients,and divided into hyperlipidemia group(n=30) and non-hyperlipidemia group(n=30).Control group consists of 30 healthy volunteers.The recipients with hyperlipidemia were treated with simvastatin for 3 months.Serum lipid levels were measured in all recipients before and 6 months after renal transplantation.Serum lipid in the recipient with hyperlipidemia was also determined after 1.5 months and 3 months of simvastatin treatment.The mRNA of bFGF and FGFR2 was detected with RT-PCR.Results Serum lipid and the mRNA expression of bFGF and FGFR2 in renal transplant recipients were significantly increased compared to healthy controls,which were more evident in hyperlipidemia group than in non-hyperlipidemia group.Serum lipid in hyperlipidemia group decreased significantly after treatment with simvastatin,meanwhile,the mRNA expressions of bFGF and FGFR2 were reduced significantly at the 1.5th month and 3rd month of simvastatin treatment,the mRNA expression of bFGF and FGFR2 reduced obviously.Conclusions The increase of the mRNA expression of bFGF and FGFR2 in renal transplant recipients with hyperlipidemia might be involved in CAN due to hyperlipidemia.Simvastatin may be used to reduce the mRNA expression of bFGF and FGFR2 in renal recipient with hyperlipidemia.
