摘要
目的建立两次打击大鼠所致的多器官功能障碍综合征(MODS)的动物模型,研究动物各器官代偿与失代偿的反应过程。方法选择清洁级SD大鼠,随机分为正常对照组(A组,32只),出血+腹腔注射内毒素组(B组,42只),出血组(C组,42只),腹腔注射内毒素组(D组,42只)。在实验的第8、24、48、72小时随机抽取每组大鼠各8只,观察各组大鼠的症状、体征、死亡率;血清内毒素血症阳性率(鲎试验);各脏器功能指标[外周血白细胞总数(WBC)、动脉血氧分压(PaO2)、丙氨酸转氨酶(ALT)、血尿素氮(BUN)、血肌酸激酶(CK)]及重要脏器的病理形态学变化。结果在实验8~24h,B组大鼠的症状、体征与功能指标(WBC、PaO2、ALT、BUN、CK)均有明显变化,与A组、C组、D组差异有显著性;其肺、肝、肾、心、脾及小肠等重要脏器病理改变明显。在48~72h,存活大鼠的症状、体征、功能指标及重要脏器病理变化逐步恢复;血清鲎试验阳性率为16.7%。MODS的发生率45.2%,总死亡率为21.4%。结论以失血加腹腔内注射内毒素的方法,可成功复制大鼠MODS模型,方法简便,能较好地反映大鼠对创伤和感染因素表现出器官功能由代偿到失代偿的反应过程。
Objective Animal model of multiple organ dysfunctions can be used to monitor the responses of organ compensation and incompensation. In this study, an animal model for multiple organ dysfunction syndrome (MODS) was genera ted by applying hemorrhagic and endotoxic shocks (two-hit model) to rats. Metho ds SD rats were randomly divided into four groups. Group A: rats received no sh ock (n=32); group B: rats were bleeding for 4 hours followed by endotoxin inject ion (i.p.) (Hemorrhagic +endotoxic shocks, n=42); group C: rats were bleeding fo r 4 hours only (Hemorrhagic shock, n=42); group D: rats received endotoxin injec tion (i.p) only (Endotoxin shock, n=42). After treatment, eight rats from each g roup were examined at the following time points: 8h, 24h, 48h and 72 h. The fol lowing parameters were tested in all groups: behaviors and symptoms, vital signs , white blood cell count (WBC), partial pressure of oxygen in arterial blood ves sel (PaO2), alanine aminotransferase (ALT), blood urea nitrogen (BUN), crestine kinase (CK), serum limulus lysate and mortality. Results Significant decreases of WBC, PaO2, ALT, BUN and CK were observed in group B compared with other group s after 8 and 24 hours of treatment (P<0.05). Severe damages were observed in lu ng, liver, heart, kidney, spleen and small intestine in group B.Positive result for serum limulus lysate test was 16.7%. All tested parameters recovered gradual ly in surviors after 48~72 hours. Percent of MODS and mortality were 45.2% and 21.4% respectively. Conclusion Hemorrhagic followed by endotoxic shock, a two-h it method, can be used to set up a simple animal model for MODS to study the res ponses of organ dysfunction coursed by ischemic and infectious diseases.
