组织工程学材料替代输尿管缺损的临床应用 被引量:9

摘要 目的总结新型替代材料修复输尿管狭窄的临床经验。方法采用组织工程学材料同种异体脱细胞输尿管细胞外基质修补治疗输尿管长段狭窄4例。男1例,女3例。平均年龄43岁。病程12~37个月,平均21个月。其中肾盂输尿管移行区狭窄1例,输尿管中上段狭窄1例,输尿管下段狭窄2例。狭窄长度3~10cm。结果4例随访11~22个月,平均17.5个月。术后4周拔除输尿管支架管1例,术后8周拔除支架管3例,无狭窄、漏尿,排尿良好。术后6个月IVU检查:输尿管造影剂通过正常,无狭窄,无渗漏。结论同种异体脱细胞输尿管细胞外基质为机体提供了良好的供宿主细胞“爬行替代”的框架,在修补后可有效地与宿主组织融合,修补后的输尿管愈合良好。 Objective To summarize the clinical experience with allodecellular extracellular matrix(ECM) for the replacement of ureter. Methods ECM was used to treat the stricture of ureter in 4 patients (1 man and 3 women; mean age,43 years).The mean disease course was 21 months (range,12-37 months).Of the 4 cases,1 had stricture of pelviureteric transition zone;1 had stricture of upper and middle ureter;and 2 had stricture of lower ureter.The stricture length ranged from 3 to 10 cm. Results The 4 cases were followed up for an average of 17.5 months (11-22 months).Postoperatively,the ureter stents were removed in 1 case at 4 weeks and in other 3 cases at 8 weeks.The ureter functions of 4 cases were normal after operation.No stenosis,effusion and hydronephrosis were observed on IVU 6 months after operation. Conclusions Allodecellular ureter ECM can provide a good frame for the host cells’ growth,and it can be mixed with host tissues to provide a good intensive potency,so it may be an ideal tissue-engineering material for the replacement of ureter.
