Expression and Clinical Significance of Survivin,Cyclin D1 and Ki-67 Proteins in Adrenal Cortex Hyperplasia and Tumors

摘要 目的 研究Survivin在肾上腺皮质的表达, 并探讨它与cyclin- D1、Ki- 67 在鉴别肾上腺皮质良、恶性肿瘤中的临床意义。方法 用免疫组化方法分析Survivin, cyclin- D1, Ki -67蛋白在8例肾上腺皮质增生、64例皮质腺瘤和24例皮质癌中的表达。结果 Survivin表达于全部标本, 但在肾上腺皮质增生、皮质腺瘤、皮质癌的表达强度有显著性差异, 表达强度≥++之标本, 在三者中分别占0%, 22%, 63%。Survivin同时表达于部分胞核, 胞核的表达可以帮助鉴别良、恶性肿瘤。cyclin D1在肾上腺皮质增生、皮质腺瘤、皮质癌中的阳性表达分别为25%, 30%和54%,Ki 67阳性表达分别为100%, 19%, 71%。结论 Survivin, cyclin- D1 和Ki- 67 的表达对肾上腺皮质腺瘤与皮质癌有一定的诊断意义。Survivin与Ki- 67两者联合表达阳性可能成为区分皮质癌与皮质腺瘤的有效方法。 Objective To investigate the expression of Survivin,cyclin D1 and Ki-67 proteins in human adrenal cortex and their clinical significance in the differential diagnosis between adrenal cortex hyperplasia and tumors.Methods The expression of Survivin,cyclin D1 and Ki-67 was detected in 8 cases of adrenocortical hyperplasia,64 cases of adrenal adenoma and 24 cases of adrenal carcinoma by immunohistochemical methods.Results Survivin was expressed in all specimens,but the expression intensity was significantly different among adrenocortical hyperplasia,adenoma and carcinoma. According to the standard of ≥++,Survivin was expressed in 0 of adrenocortical hyperplasia,22% of adrenocortical adenoma and 63% of adrenocortical carcinoma respectively. Survivin was expressed in some nuclei,and this kind of expression could help to distinguish adenoma from carcinoma. The positive expression rate of Cyclin D1 was 25%,30% and 54% in adrenocortical hyperplasia,adenoma and carcinoma respectively,while that of Ki-67 was 100%,19% and 71% respectively.Conclusion Detecting the expression of Survivin,cyclin D1,and Ki-67 had some diagnostic values in adrenocortical adenoma and carcinoma. The combined detection of Survivin and Ki-67 may be an effective way to distinguish carcinoma from adenoma in adrenal cortex.
