伽玛刀治疗头部疾病的初步报告

Primary Report Treatment of Extra and Intracranial Lesions with Leksell Gamma Knife

摘要 上海医科大学华山医院于1993年10月至1994年1月用伽玛刀治疗了141例颅内外疾病(共146个病灶),其中有动静脉畸形(AVM)12例、脑膜瘤34例、听神经瘤17例、胶质瘤25例、松果体区肿瘤14例、转移瘤16例及其他病变等。患者年龄7~78岁,平均43岁。19例伽马刀术后1~3个月作了影像学随访,其中4例转移瘤5个病灶缩小或消失,3例松果体区肿瘤明显缩小,4例脑膜瘤病灶大小无甚改变,但神经系统病损改善,2例鼻咽癌咽鼓管阻塞症状改善。所有病例未见严重并发症。我们的经验及短期随访结果显示:在AVM和颅内外肿瘤和选择性病例中伽玛刀对控制肿瘤和改善症状是一种有效的治疗手段。 Between Oct.1993 and Jan.1994,14 6 extra and intracranial lesions were treated with Leksell gamma knife in Hua-shan hospital, Shanghai medical university. The patients aged from 7 to 78 years old (mean 43 years).There were in 12 AVM, 34 menigiomas,17 acoustic neuromas,25 glioma, 14 pineal region tumors,16 metastatic tumors and others. The average largest diameters of fo-cus(x±s) were:AVM 21.2±1. 0mm,menigioma 35.6± 13.7mm,acoustic neuroma 27.0±6. 6mm,glioma 31.9±14.8mm,pineal region tumor 24.7±7.4mm, metastases 25.3±12.3mm,pituitary ade-noma 23.5±10.9mm, nasopharyngeal carcinoma 38.0± 9.9mm,craniopharyngioma 36. 5± 15.8mm,and others 31.8±15.9mm. The average central maximal dose and average marginal dose (x ± s Gy)were in AVM 44.75±6.5,19.5±2.6;in menigioma 40. 78±6.6,13.62+4.3; in acoustic neuroma 34.6±5.4,12.5±2.5; in glioma41.2±8.5,15.6±17.7,24.6±4.27; in pituitaryadenoma46.5±11.5,22.5±11. 3;in nasopharyngeal carcinoma 52±4.5,22. 6±3. 9;in cranio-pharyngioma 33±1.4,14.5±3. 5 and in others 43.8±5.8,17.2±3.8.Nineteen patients were followed for 1~3months after the gamma knife treatment metastatic le-sions in 4 cases were red uced in size or d isa ppeared;3 cases of pineal region t umor were significant ly smaller lesions;4 cases of menigioma failed to demonstrate change in diameter of lesion however the but neurological deficits improved.2 cases of nasopharyngeal carcinoma showed improvement of their audi-tory tude obstructive symptoms. No severe complications were found in this group. Our experience and the short term follow-up results showed that gamma knife is a useful tool in tumor control and symp-tomatic relief for patients with AVM and selective patients with extra and intracranial tumors.
