摘要
本研究为单盲随机安慰剂对照多中心临床试验，评价钙拮抗剂─－硝苯地平治疗高血压治疗效果及安全性。在１９８６年６月～１９９２年６月，观察６８３例（治疗组３４１例，对照组３４２例），观察时间３．５年至６年，平均５８个月，平龄４０～７０岁。治疗组于用药后第１、３、５年平均血压下降，均明显大于对照组，心率治疗组稍增加，非心血管事件两组相近，治疗组较对照组总心血管意外降低４１．７％，总心血管死亡降低２０％，冠心病发生降低１４．２％，卒中发生降低５０％。影响心血管事件发生的因素有年龄、基础血压、降压幅度、心律失常、基础心率、血糖、右房内径。室间隔与室壁厚度。硝苯地平用药３年对总胆固醇、血糖、低密度脂蛋白、高密度脂蛋白有轻度影响。顺从性是治疗转归极重要的决定因素之一，影响顺从性因素有年龄、文化、疾病严重程度等。
Abstract A multicenter randomized. placebo controlled.single-blind intervention trial was performd in 683hypertensives (341 in nifedipine group, NG, 342 in the placebo group, PG) to assess the impacts of longterm nifedipine treatment on the cardiovasular events and total death from June 1986 to June 1992 with a mean follow-up period of 58 months. The results showed that the cumulative cardiovascular events in NG were lowered by 41.4%(P<0.05) than it in the PG (5.2% vs 14%). The incidences of stroke were reduced 50% in NG (P < 0.05)than in PG (4.1% vs 8.2%). The incidence of ischemic heart disease and the car. diovascular death for NG and PG were 2.4%, 2.9%; and 1.8%, 2.1%,respectively. There were no significant differences in incidence of cancer and noncardiovascular death. The conclusion is that nifedipine was efficacious.safe. well tolerated in the longterm treatment of hypertension With a benificial impact on the serum cholesterol, LDL and LVH. The total cardiovascular events and the incidence of stroke were decreased significantly. No significant difference can be found in the cardiovascular death, incidence of CAD, and the noncardiovascular death in two groups. The factors related to the prognosis and the importance of compliance of hypertensive subjects were discussed.
