氧饱和度监测结合ESS评分对阻塞型睡眠呼吸暂停低通气综合征的诊断价值 被引量:2

摘要 目的:探讨ESS评分及夜间氧饱和度监测对阻塞型睡眠呼吸暂停低通气综合征(OSAHS)患者的诊断价值。方法:对比124例打鼾者的ESS评分、夜间氧饱和度监测及多导睡眠图结果,并根据呼吸紊乱指数(RDI)将其分为单纯打鼾、轻度OSAHS、中度OSAHS和重度OSAHS组进行分析。结果:单纯鼾症、OSAHS各组患者ESS评分、RDI、最大氧减幅度、氧减指数差异有显著性。氧减指数与RDI均密切相关。以ODI≥5诊断OSAHS的敏感性为96.9%,特异性为78.6%,以ODI≥5结合ESS≥5来诊断SAS的敏感性为92.7%,特异性为96.4%。结论:选用合适的氧减指数并结合患者的ESS评分对诊断OSAHS有一定的价值。 Objective: To explore and discuss the the diagnotic value of ESS scale and nocturnal oxygen saturation monitor for the patient of OSAHS. Methods: One hundred and twenty-four snorers compared with Epworth Sleepiness Scale (ESS),nocturnal oxygen saturation monitor and result of polysomography (PSG) were divided into simple snore,slight OSAHS,moderate OSAHS,and severe OASHS according to respiratory disturbance index (RDI) and analysed. Results: There were significant difference among ESS,RDI,the largest oxygen desaturation range (LODR) and oxygen desaturation index (ODI) in patients of each group of simple snore and OSAHS. There was statistical significant correlation (r=0.919,P<0.01) between ODI and RDI. The sensitivity of OSAHS was 96.9%,but the specifity was 78.6% when ODI≥5 was regarded as a diagnostic standard. The sensitivity of SAS was 92.7%,the specificity was 96.4% when ODI≥5 combined with EES≥5 was regarded as the diagnostic standard. Conclusions: Selection of appropri- ate ODI combined with Epworth Sleepiness Scale has definite value for the diagnosis of OSAHS.
