血氧深入图像对室间隔缺损术后患者肺氧摄取及组织氧利用的评估

Deep picture blood gas analysis for evaluation of the oxygen uptake in lung and the oxygen utilization in tissue of postoperative patients with ventricular septal defect

摘要 目的:探讨血氧深入图像分析在室间隔缺损术后评价肺氧摄取、血氧运输、组织氧利用中的价值。方法:采用深入图像血气分析方法对2004-01/06解放军沈阳军区总医院住院治疗的室间隔缺损肺动脉高压患者50例和室间隔缺损无肺动脉高压患者50例术后2,12,24h进行动脉血气分析,比较两组术后的深入图像血氧值。结果:每组50例进入结果分析。肺动脉高压室间隔缺损组术后2,12和24h氧分压和肺泡动脉氧分压差值与无肺动脉高压室间隔缺损组无明显差异(P>0.05);组内各时间点比较差异不明显(P>0.05)。两组血红蛋白总量、氧合血红蛋白水平组间及组内比较,差异均无显著性(P>0.05)。肺动脉高压室间隔缺损组血红蛋白半饱合度和组织耗氧量各时段低于无肺动脉高压室间隔缺损组,差异有显著性(P<0.01)。结论:深入图像血气分析可提供肺氧摄取、血氧运输、组织氧利用等指标,并可反映出室间隔缺损肺动脉高压术后在氧分压正常的情况下组织氧利用不良。 AIM:To study the value of deep picture blood gas analysis in evaluating oxygen uptake in lung,oxygen transport in blood and oxygen utilization in tissue in postoperative patients of ventricular septal defect.METHODS:Arterial blood gas analysis was undertaken in 50 patients with ventricular septal defect and pulmonary hypertension and 50 patients with ventricular septal defect but without pulmonary hypertension,who were hospitalized in the Chinese PLA General Hospital of Shenyang Military Area Command between January and June 2004,with the method of deep picture blood gas analysis 2,12 and 24 hours postoperation,and the postoperative values of deep picture blood gas were compared between the two groups.RESULTS:Fifty cases in each group were involved in the analysis of results.There were insignificant differences in the partial pressure of oxygen and difference value of partial pressure of oxygen in pulmonary alveolus 2,12 and 24 hours postoperation between the ventricular septal defect patients with or without pulmonary hypertension(P >0.05);The differences were not obvious at each time point in the same group(P >0.05).The total amount of hemoglobin and level of oxyhemoglobin were not significantly different both between the two groups and at each point in the same group(P >0.05).The semi saturate of hemoglobin and oxygen consumption in tissue at each time point were significantly lower in the patients with ventricular septal defect and pulmonary hypertension then in those without pulmonary hypertension(P< 0.01).CONCLUSION:The deep picture blood gas analyzis can offer the indexes of oxygen uptake in lung,oxygen transport in blood and oxygen utilization in tissue,and it can reflect that the oxygen utilization in tissue is poor under normal partial pressure of oxygen in postoperative patients with ventricular septal defect and pulmonary hypertension.
