^(99)Tc^m-奥曲肽眼眶显像在甲状腺相关眼病中的初步应用 被引量:1

Application of orbital scintigraphy with ^(99)Tc^m-octreotide in patients with thyroid associated ophthalmopathy

摘要 目的应用99Tcm-奥曲肽眼眶显像判断甲状腺相关眼病(TAO)活动度的价值。方法选16例TAO患者,按照临床活动度评分(CAS)分为活动组(AG)和非活动组(IAG),并选5名健康志愿者为正常对照组(CG)。注射99Tcm-奥曲肽740MBq,1h后行头颅断层显像。枕部作为校正因子,对三组眼眶的奥曲肽摄取率(UR)进行方差分析,并对URall和CAS进行秩相关分析。结果三组UR分别为URAG=1.44±0.21、URIAG=1.11±0.10和URCG=1.11±0.06,URAG与URIAG、URAG与URCG相比,具有显著性差异(P<0.05)。URIAG与URCG相比,无统计学意义(P>0.05)。URall与其CAS具有良好的相关性,rs=0·91。结论99Tcm-奥曲肽的眼眶显像可用以判断TAO患者眼部的炎性反应程度。 Objective To analyze the value of estimation activity of thyroid associated ophthalmopathy (TAO) with orbital scintigraphy. Methods There were 16 patients with TAO and Graves' disease (GD), and 5 volunteers (control group, CG) without eye disease or GD. All patients were divided into 2 groups by the clinical activity score (CAS):activity group (AG) and inactivity group (IAG). SPECT studies of skull were completed within 1 h after iv. 740 MBq  99Tcm-OCT. The region of interest (ROI) of right orbit (R), left orbit (L) and occiput (O) were drawn. The uptake ratios (UR) were calculated in (R+L)/2O. The statistical analyses included ANOVA and rank correlation using SPSS 10.0. Results The orbit appeared as a “cold area” in CG. The orbit uptake of AG had a visual orbital accumulation of  99Tcm-OCT, but little in IAG (UR AG=1.44±0.21, UR IAG=1.11±0.10 and UR CG=1.11±0.06). There was a significant difference between IAG and AG (P<0.05). A statistically significant correlation was found between CAS and UR (rs=0.91). Conclusion  99Tcm-OCT scintigraphy may be useful to estimate a positive response activity of TAO.
