摘要
目的:探讨智能低下儿童主要照顾者情绪障碍的发生、表现及相关影响因素。方法:采用Gesell儿童发育量表对31例精神发育迟滞患儿进行测查,全面评价其发育情况,并对其主要照顾者进行医院焦虑抑郁量表(HAD)和社会支持评定量表(SSRS)、自行设计的一般情况调查表和情绪障碍调查表测查,运用多因素多水平分析方法分析精神发育迟滞儿童照顾者情绪障碍的影响因素。结果:①31例智能低下儿童的主要照顾者中,约80%存在焦虑、抑郁情绪倾向。②智能低下儿童照顾者的情绪障碍,与患儿是否有异常行为倾向、是否接受系统的康复训练以及照顾者与小儿的关系有关(P<0.05,P<0.01,P<0.05)。③多因素、多水平回归分析表明:Gesell发育量表中语言领域得分、SSRS社会支持总分、客观支持维度得分3个因素与照顾者情绪障碍呈显著负相关(P<0.05,P<0.01,P<0.05)。结论:智能低下儿童主要照顾者的情绪障碍应受到关注,干预对象应扩展到患儿的主要照顾者,甚至整个家庭。
Objective: To study the clinical characteristics of emotional disorder on caregivers of children with mental retardation and analyse its influence factors. Method: 31 caces of MR children were assessed by Gesell developmental scales and their caregivers were investigated by HAD and SSRS, meanwhile they were inquired with general circumstances questionnaire and emotional disorder questionnaire designed by ourselves. Result: ①About 80% of caregivers were depressive and anxious, much more than ordinary people. ②The emotional disorder of the caregivers was relative to behavioral deviation of the children, rehabilitation treatment and relationship between caregiver and the child(P<0.05,P<0.01,P<0.05). ③Multi-factor and multi-level analysis indicated that the emotional disorder was significantly negative related to three factors of the score of language's domain on Gesell developmental scale for children, the total score of social support rating scale and the score of objective support dimension for caregiver(P<0.05,P<0.01,P<0.05). Conclusion: Emotional disorder of the caregivers for MR children should be concerned. The object of intervention should be expanded to the caregivers, even the whole family.
