阻塞性通气障碍的CT肺功能成像参数 被引量:15

Parameters of CT pulmonary function imaging in emphysema and obstructive ventilatory defect

摘要 目的 建立诊断肺气肿及阻塞性通气障碍的CT肺功能参数阈值。方法 对照组20 例,阻塞性通气障碍组37例(包括肺气肿23例、哮喘6例、不明原因8例)。肺功能检查和定量CT检查均在1周内完成。利用CT肺功能定量分析软件分析图像数据并得出相应的肺功能参数值,包括肺平均CT密度值、像素指数(PI-700^-800,PI-800^-900, PI-910,PI-950),比较各组参数值的差异。结果 阻塞性通气障碍组最大吸气末CT值为(-834.68±29.54) Hu,最大呼气末CT值为(-770.48±55.07) Hu,与正常组均存在差异。吸气末及呼气末PI-910in、PI-910ex与PFT各参数(FEV1,FEV1/FVC,DLco%)的相关性分别较其他像素指数要好。PI-910in>15%考虑患有阻塞性通气障碍, PI-950in、PI-910ex的分布代表阻塞性通气功能障碍的范围,PI-950ex代表肺气肿的分布范围。结论 建立CT肺功能成像参数阈值可为临床诊断相关疾病提供客观依据。 Objective To establish CT the diagnostic threshold value of pulmonary functional for emphysema and obstructive ventilatory defect. Methods The study included control group with 20 normal volunteers and obstructive ventilatory defect group with 37 patients (23 with emphysema, 6 with asthma and 8 with unknown etiology). Enroller performed both the inhale-exhale helical CT examination and pulmonary function test within one week. All CT images were analyzed by CT pulmonary function quantitative software. The obtained functional parameters included mean lung CT density, pixel indexes (PI_ -700~-800 , PI_ -800~-900 , PI_ -910 , PI_ -950 ), and were compared among groups. Results The mean lung CT value of obstructive ventilatory defect obtained at full inspiratory and full expiratory was (-834.68±29.54) Hu and (-770.48±55.07) Hu, respectively. Significant difference of mean CT values was seen between the control group and case group. PI_ -910in at inspiratory CT and PI_ -910ex at respiratory CT respectively showed better correlation with PFT parameters than other thresholds. At inspiratory CT, PI_ -910in exceeding 15 percent represented obstructive ventilatory defect, PI_ -950in and PI_ -910ex were used to quantify the field of obstructive ventilatory defect, PI_ -950ex was used to display emphysema distribution. Conclusion Establishing diagnostic threshold value of emphysema and obstructive ventilatory defect may be useful for quantitative assessment of relevent pulmonary diseases.
