摘要
目的探讨本地区中心性浆液性脉络膜视网膜病变(简称中浆)的流行病学规律。研究激光治疗与药物治疗(复方樟柳碱注射液、VitC、VitB1、VitP)在缩短病程、视力恢复及预防复发方面的差异。方法选取门诊就诊怀疑中浆的患者,全部给予眼底检查及眼底荧光血管造影以明确诊断共167例,并给予问卷调查,根据FFA将渗漏点在黄斑中心250μm以外的127例随机分为3个治疗组,A组:激光治疗,采用法国BVI-532激光器,41例;B组:复方樟柳碱注射液颞浅动脉旁皮下注射,43例;C组:VitC、VitB1、VitP口服,43例;渗漏点在黄斑中心250μm以内的40例列为D组。某矿区2300人进行问卷调查,中浆患者21例,患病率9.1/10000。结果中浆多发于20～50岁的青壮年,男:女=3.08:1,多见于农民、工人(野外工作者),一年四季均可发生,发病诱因多为睡眠不足、情绪波动,多数人有烟酒嗜好。激光治疗组在症状消失时间及眼底检查神经上皮脱离消失时间上与另二组相比有显著性差异(P<0.05),激光治疗组和复方樟柳碱治疗组在视力恢复至患病前所需时间上,及FFA检查渗漏点消失时间上无差异(P>0.05),但较VitC、VitB1、VitP治疗组有显著性差异(P<0.01)。3组复发率比较无显著性差异。结论中浆是本地区的多发病,激光治疗及复方樟柳碱治疗均能明显缩短病程,利于视力早日恢复,但均不能降低复发率。
Objective To discuss the epidemiology rule of local central serous chorioretinopathy, so as to offer evidences for prevention. To study the differences on course decurtation, vision recovery and occurrence prevention among laser therapy, medical therapy(compound anisodine injection/Vitamin C, Vitamin B_1,Vitamin P).Methods The outpatients doubted as central serous chorioretinopathy were given fundus fluorescein angiography(FFA) to definitude diagnosis and questionnaire investigation. They were divided into three treatment groups randomly: Group A-laser therapy by French BVI-532 laser; Group B-compound anisodine injection by hypodermic injection beside superficial temporal artery; Group C-vitamin C, vitamin B,vitamin P by oral administration.Results Central Serous Chorioretinopathy usually occured among those aged 20-50, the ratio between male and female was 3.08:1. Most of the patients were peasants, workers(outdoor operators). It may occured in all four seasons of the year. The inducement was mostly due to lack of sleep, hyperemotivity and tobacco alcohol addiction. As for the symptom disappearance time and neurepithelium exfoliation disappearance time by fundus examination, Group A had a great difference compared with the other two groups (P<0.05). Group A had no great difference compared with Group B on vision recovery time and FFA examination diagnosis point disappearance time but had a great difference compared with Group C (P<0.01). As for the occurrence rate, there was no geeat difference among the three groups.Conclusion Central Serous Chorioretinopathy was a frenquently encountered disease in our location. Laser therapy and compound anisodine injection can both decurtate courses apparently and can be propitious to vision recovery at an early date. But they both can't reduce hte occurrence rate.
