摘要
目的检测粪肠球菌和屎肠球菌的耐药表型及多种耐药基因。方法琼脂稀释法测定抗菌药物对肠球菌的最小抑菌浓度(MIC);聚合酶链反应(PCR)检测tetM、ermB、TEM、mefA、aac(6′)aph(2″)、ant(6)Ⅰ基因。结果屎肠球菌对氨苄西林、青霉素G、红霉素、环丙沙星、左氧氟沙星、青霉素的耐药率分别为94.1%、100%、94.1%、88.2%、94.1%、85.3%,均高于粪肠球菌的12.5%、59.4%、78.1%、43.8%、68.8%、18.8%;对氯霉素的耐药率17.6%低于粪肠球菌的40.6%,但中介率已达64.7%,高于粪肠球菌中介率46.9%;肠球菌高水平庆大霉素耐药(HLGR)和高水平链霉素耐药(HLSR)检出率高,屎肠球菌中HLGR、HLSR的比例88.2%、67.6%分别高于粪肠球菌的68.8%、50.0%;全部肠球菌对万古霉素敏感。肠球菌中耐药基因检出率高,屎肠球菌中ermB、TEM、aac(6′)aph(2″)、ant(6)I分别为76.5%、26.5%、91.2%、64.7%,高于粪肠球菌的62.5%、0.0%、75.0%、62.5%;tetM的检出率38.2%低于粪肠球菌的43.8%;全部肠球菌未检出mefA基因。结论肠球菌对多种抗菌药物耐药率较高,屎肠球菌耐药比粪肠球菌严重。
Objective To detect the drug-resistant phenotype and multidrug-resistant genetype in enterococcus faecalis (E.faecalis) and enterococcus faecium (E.faecium).Methods Minimum inhibitory concentration (MIC) of 10 antibiotics on enterococcus was determined by agar dilution method.The genes,tetM,ermB,TEM,aac(6′)-aph(2″) and ant(6)-I,were detected by polymerse chain reaction (PCR).Results The resistance rates of ampicillin,penicillin G,erythromycin,ciprofcoxacin,levofloxacinand penicillin in 34 E.faecium were higher than those in 32 E.faecalis.The resistance rate of chloramphenicol was lower than that in E faecalis but the 64.7% of intermediate proportion was bigger than 46.9% in E faecalis.High-level gentamicin resistance (HLGR) was 88.2% and high-level streptomycin resistance (HLSR) was 67.6% in E.faecium,which were higher than 68.8% and 50.0% in E faecalis respectively.All enterococci were susceptible to vancomycin.High rates of antibiotic resistance genes were found.The detectable rates of ermB,TEM,aac(6′)-aph(2″) and ant(6)-I in E faecium were higher than those in E faecalis.The detectable rate of tetM was 38.24% which was lower than 43.75% in E faecalis.No mefA was detectable in our study.Conclusions High rates of antibiotic resistance for multiple antibiotics were found in enterococcus spp.The antibiotic resistance in E faecium is more serious than that in E faecalis.
