摘要
目的观察丝裂霉素C对兔后发性白内障形成的影响及对兔眼的毒性作用,为临床上先天性白内障及青少年发育性白内障提供一种有效的治疗方法.方法以兔眼为模型,取20只新西兰家兔在常规手术显微镜下行晶状体囊外摘除后分别注入0.02%丝裂霉素C 0.15ml,然后观察术后不同时间角膜及晶状体后囊混浊变化,并结合观察术后病理学和电镜结构变化.结果 10只兔眼注入丝裂霉素C后1周炎性反应比对照组稍重,有轻度的房水混浊,内皮无明显水肿;3~5周后房水清晰,后囊膜明显比对照组清晰.组织病理学检查表明:对照组术后晶状体上皮细胞增生,而用药组明显抑制晶状体上皮细胞增生,并且对角膜无明显毒性作用.丝裂霉素C引起晶状体上皮细胞空泡变性、核固缩等,电镜观察发现用药组晶状体上皮细胞体积缩小、胞浆浓缩、染色质靠边聚集、包膜完整等典型的细胞凋亡形态.结论丝裂霉素C能有效地抑制晶状体上皮细胞的增殖,预防兔后发性白内障的形成,且对兔眼毒性小,是一种有效的防治后发性白内障的药物.为以后开展临床治疗先天性白内障及青年性白内障时预防后发障提供了可靠的基础,目前已开始初步应用于临床.
Objective To investigate the effect of as therapeutic agent whether it is beneficial in prevention of posterior capsular opacification(PCO)and the toxicity of the drug.Methods Twenty rabbit eyes were injected 0.15ml mitomycin solution 0.02% mitomycin C into the capsular bag during lens extracapsular extraction respectively.The posterior capsule and keratoderma were observed by biomicroscopy in different times.The histopathological changes and transmission electronic microscope examination were also studied.Results In the first week after the injection of mitomycin C into twenty rabbit eyes,the inflammatory response was a little severer than that in the control group.The cyst membranes were much clearer than those in the control group from the third week to the eighth week after the operation.Histopathological study,revealed that lens epithelial cells proliferated developed in the control eyes.However,only mild vacuolization and pkynotic change of lens epithelial cells were noted in the mitomycin C treated eyes.Transmission electronic microscope demonstrated typical morphological appearance of apoptosis,the cells shrank,chromatin condensation and the margintion.Conclusion Mitomycin C can inhibit metaplasia and proliferation of lens epithelial cells in rabbit eye without obvious toxicity.It can be used as a potential agent in the prevention of the development of PCO.
