摘要
使用波长1.06um的Nd:YAG激光(光纤直径600um),于1993年3月将经皮激光椎间盘减压术(PLDD)应用于临床。首批治疗腰椎间盘突出症10例,均随访6个月;按Macnab疗效评定标准,结果为优7例、良3例,即刻及近期疗效满意。术中采用实时超声监视激光辐射汽化过程,有助于提高操作的精确性和安全性。另就PLDD技术的治疗作用机理、病例选择、此技术的优越性、激光辐射要素、实时超声监视的特点及疗效等问题进行讨论。
n the basis
of our previous experimental studies,the technique of PLDD was
introduced in cli-nical practice in March of l993.10 patients
underwent PLDD utilizing Nd:YAG laser(1.06μm wave-length)with a 600
μm quarz fiber.The mean follow-up period was 6 months.According to
theMacnab’s criteria,excellent response Was achieved in 7 cases,and
good response in 3 cases.Based uponthis early result,we conclude that
PLDD with Nd:YAG laser(1.06μm)is a safe,effective and lessinvasive
therapeutic modality f0r contained lumbar disc herniation,provided
patients were carefullyselected.We also consider that intra-operative
real-time ultrasonic monitoring is a simple and effectivemethod.The
therapeutic mechanism,selection of patients, advantages of PLDD,main
points of laserradiation,characteristics of real-time ultrasonic
monitoring and success rate were also discussed in thearticle.
