摘要
目的探讨99mTcO4-涎腺动态显像对干燥综合征(SS)的诊断价值。方法采用MarconiFx810双探头SPECT仪对30名正常志愿者和34例临床确诊的SS患者进行99mTcO4-涎腺动态显像,利用感兴趣区技术生成双侧涎腺时间—放射性曲线,计算摄取指数(UI)、排泌指数(EI)等功能参数,并将SS患者各项功能参数与唇黏膜病理学分级进行相关性分析。结果SS患者涎腺UI、EI显著降低,与对照组比较差异有统计学意义(P<0.01)。腮腺及颌下腺摄取功能指标与病理分级有良好的相关性(r值分别为-0.311,-0.321;P值为0.01,0.008)。腮腺EI值与病理分级无统计意义的相关性,Ⅱ ̄Ⅲ级EI值相近,颌下腺EI值与病理分级无明显相关性,且与正常组有部分交叉。结论99mTcO4-涎腺显像定量分析对SS诊断的灵敏度高,对病情评估、疗效观察、预后判断等均具有重要意义。该方法操作简便、无创、重复性好,值得临床推广应用。
Objective To determine the value of quantitative salivary gland imaging with pertechnetate-^(99m) (^(99m)TcO4-) in the diagnosis of Sjgren′s syndrome (SS). Methods Model marconi FX810 SPECT machine and dynamic pertechnetate-99m salivay gland imaging were used to determine the uptake index (UI) and excretion index (EI) of salivary gland in the control group of 30 nomal volunteers and in the experimental group of 34 cases of SS. All patients with SS underwent labial biopsy and Spearman rank correlation test as used for scintigraphy functional parameters and histopathologic grading. Results In patients with SS, the UI and EI were significantly lower than that in the control group (P<0.01). The UI of salivary gland was significantly decreased with histopathologic grading incease (r=-0.311 and -0.321 P<0.01). The EI of Par gland decreased in early SS, although it was not correlated significantly with histopathologic grading. Conclusion Salivary gland scintigraphy is an easy and noninvasive method for evaluating patients with SS.
