摘要 目的观察亚麻醉剂量安定-氯胺酮对烧伤小鼠早期肝功能及死亡率的影响,初步探讨安定-氯胺酮在严重烧伤早期救治中的作用.方法 200只雄性BALB/c小鼠,随机分为正常对照组、单纯烧伤组、麻醉对照组、预处理组、后处理组.采用安定-氯胺酮亚麻醉,背部15%~20%Ⅲ度烧伤.测定4 h丙氨酸转氨酶(ALT)和门冬氨酸转氨酶(AST),并观察72 h内小鼠的死亡情况.结果单纯烧伤组血清ALT、AST显著高于正常对照组,预处理组、后处理组也较正常对照组明显升高,但显著低于单纯烧伤组,后处理组显著高于预处理组.单纯烧伤组死亡率明显高于其他各组,预处理组小鼠死亡率显著降低,后处理组死亡小鼠也明显减少,与预处理组相差不显著.结论严重烧伤后早期肝功能明显受损,死亡率增加;严重烧伤前后使用亚麻醉剂量安定-氯胺酮明显减轻肝功能损害,降低死亡率. Objective To study the effect of subanesthesia diazepam-ketamine on liver function and mortality in burned mice.Methods 200 BALB/c male mice were divided randomly into five groups:normal control,anesthesia control,burn,pretreatment and posttreatment.Mice were produced 15%~20% thickness third degree burn injury and received diazepam-ketamine subanesthesia.Pretreatment or posttreatment group received diazepam-ketamine initiated 15 min before or after burn injury. The concentrations of serum alanine aminotransferase(ALT)and aspartic acid+aminotransferase(AST)were detected at 4 h after burn,and the mortality was observed during 72 h after burn or anesthesia.Results Serum ALT and AST in burn group were markedly elevated compared with normal control group.The level of serum ALT and AST in pretreatment and posttreatment group were also markedly higher than those of normal control group, but obviously lower than those of burn group.The concentrations of ALT and AST of posttreatment group were significantly higher than those of pretreatment group.The mortality of burn group was 20% at 30 min,50% at 4 h,70% at 12 h and 77% at 24 h and markedly elevated compared with other groups.In pretreatment group,the mortality (0 at 2 h,only 3%at 4 h,7% at 12 h and 10% at 24 h) was obviously decreased compared with burn group.It was also markedly reduced in posttreatment group,but there was no significant difference between posttreatment group and pretreatment group.Conclusion These results indicate that severe burn injury induces progressive liver functional impairment and increased mortality.Diazepam-ketamine subanesthesia could relieve liver funtional impairment and reduce the mortality when it was used before of after burn injury.
