摘要
本文报道了应用超声心动图面积-长度法评价15例闭塞性肺血管病右心功能及吸入一氧化氮(NO)后右心功能的变化。结果表明右室舒张末期容积(RVEDV)和收缩末期容积(RVESV)增加,右室每搏量(RVSV)、右室射血分数(RVEF)和右心排血量(RVCO)均降低。吸入NO后收缩期跨三尖瓣峰值压差平均降低23%(P<0.001),RVEDV和RVESV分别减小17.7%和37.4(P值均小于0.001),RVSV、RVEF、RVCO分别增加30.8%、58.7%、34.4%(P值均小于0.01-0.001),提示右心功能有明显改善。
To evaluate right ventricular function in patients with obliterative pulmonary vascular diseases and effect of inhaled nitric oxide,fifteen cases of obliterative pulmonary vascular diseases were studied with echocardiographic method of the area-length,including eight patients with primary pulmonary hypertension and seven patients with pulmonary embolism. Results showed right ventricular end diastolic volume (RVEDV)and end systolic volume(RVESV)were increased.Stroke volume,ejection fraction,cardiac output of the right ventricle(RVSV,RVEF,RVCO)were all decrased. After inhaled nitric oxide,systolic mean peak pressure gradient across tricuspid valves was decreased 23%.RVEDV and RVESV were decrease 17%(P<0.001)and 37.4%(P<0.001),separatively;RVSV,RVEF and RVCO were increased 30.8%,58.7%and 34.4%(P<0. 01-0.001),separatively.It indicated that the function of the right heart was improvedapparently.
