摘要
目的探讨不同病因引起的原发性甲状腺功能减退(简称甲减)的声像图特点。方法用彩色多普勒超声诊断仪观察甲状腺大小、内部回声、血流分布。结果本组116例经临床诊断和实验室证实为甲减的患者中,40例缺碘引起的亚临床甲减、亚急性甲状腺炎、甲状腺肿大型桥本氏甲状腺炎表现为甲状腺不同程度的肿大,其峡部增厚程度与甲状腺功能亢进(简称甲亢)组比较差异有极显著性(P< 0.001),内部回声减低为其特征性表现,彩色多普勒显示(CDFI)实质内部血流丰富类似于甲亢表现。另21例缺碘引起的亚临床甲减患者, 甲状腺内部见单发或多发结节,甲状腺肿大程度和内部回声、血流分布情况无特征性表现。12例亚急性甲状腺炎表现为甲状腺内局限性低回声团块伴丰富的血流信号。21例腺体缩小型桥本氏甲状腺炎表现为甲状腺体积缩小,甲状腺上动脉流速减慢,与正常组比较差异有显著性(P<0.001),内部回声增粗增强。13例甲亢治疗后引起甲减患者其声像图无特异性表现。9例甲状腺肿瘤次切术后引起甲减患者声像图表现为残甲内回声减低,血流丰富。结论甲减因病因不同而有不同的声像图表现。
Objective To investigate the sound - graphic feature in primary hypothyroidism origining from different causes. Methods The size of thyroid, its inside echo and blood flow distribution were observed by color doppler flow imagion(CDFI). Results Among 116 cases of hypothyrordisms diagnosed through clinical and laboratory, 40 cases of subclimcal hypothyroidism, subacute thyroidism and enlarged Hashimoto's thyroiditis caused by insufficient Iodine exhibited with different degree of thyroid enlargement and significant difference in the thickens of isthmus compared with hyperthyroidism( p < 0.001), their characteristic exhibition were weaken on echo and inside blood flow by CDFI which similar to hypothyroidism. In 21 cases of subclinical hypothyroidism derived from insufficient Iodine, there were single or multi - nodules in thyroid, and no characteristic manifestation, in the enlargement degree of thyroid, inside echo and distribution of blood flow . In 12 cases of limited acute thyroiditis there were local low echo mass with nch blood flow signal. In 21 cases of Hashimoto's thyroiditis, there was significant difference compared with normal group (p < 0.001) in the volume of thyroid, upper artery blood flow . There were no characteristic sound - graphy manifestation in 13 cases of hypothyroidism followed of treatment of hyperthyroidism. The residual thyroid of 9 cases of hypothyroidism after thyroidectomy were manifested with low echo and rich blood flow in sound - graphy. Conclusion Hypothyroidism derived from different causes was exhibited with different sound - graphy.
