血清胱抑素水平在肝硬化患者检测中的价值 被引量:4

摘要 目的为探讨血清胱抑素在肝硬化患者肾功能损害的应用价值。方法20名健康对照及76例肝硬化患者(Child-Pugh积分分组:A级16例、B级37例、C级23例),同时标本采样检测Ccr;留取24h尿液标本,同时采静脉血3mL,分别检测血肌酐和尿肌酐,并换算内生肌酐清除率(换算公式:Ccr=尿肌酐浓度(μmol/L×尿流量(mL/min)×1.73m2/血肌酐μmol/L×体表面积m2)。设备日丽7600-010型全自动生化分析仪及德灵全自动特定蛋白(Bnprospec分析)分析仪,血cys.C采用Behring免疫散射比浊法。结果1A\B\C血清胱抑素均比正常组高,4组cys.C检测值差异有显著性(P<0.05)。在肝硬化组与对照组内生肌酐清除率与血胱抑素水平构成比显示cys.C检查的阳性率高于Ccr,二者差异有显著性(P<0.05),用相关性分析肝硬化病人血清胱抑素C与Ccr异常之间有显著负相关(r=-0.452,t=4.13(76)0.01,P<0.01)。结论胱抑素测定是反映肝病患者的肾功能状况良好且快速简便的指标。
