摘要 目的:探讨CT灌注成像在胰腺癌诊断中的作用。方法:胰腺癌患者39例,急性胰腺炎患者12例,正常胰腺8例,以4.0ml/s的速度团注造影剂50ml后8s,采用SiemensBodyperfusion体部灌注成像序列,对选定部位相邻两层进行动态增强扫描。动态增强扫描结束后追加造影剂50ml(2ml/s)进行常规胰腺扫描,扫描分为动脉期、胰腺期,分别为注射造影剂后25s、45s。动态增强图像用SiemensPerfusionCT/VA11A软件处理,生成256色彩色灌注图,测量血流量、血流容积、无血管血流量、组织强化峰值、灌注起始时间和峰值时间值。结果:胰腺癌的血流量、血流容积、强化峰值较急性胰腺炎、正常胰腺低(P<0.01);灌注起始时间3组之间无统计学差别;胰腺癌较急性胰腺炎灌注峰值时间长(P<0.05),与正常胰腺无差别;彩色灌注图直观、方便,在病变的定性诊断方面有一定作用,但如果要定量分析,仍需测量灌注值。采用SiemensPerfusionCT/VA11A软件得到的正常胰腺灌注值为(0.84±0.32)ml/(min·ml)。结论:胰腺癌的血流量、血流容积、强化峰值等血流动力学指标较急性胰腺炎、正常胰腺的普遍下降;彩色灌注图直观、方便,在病变的定性诊断方面有一定作用。 Objective: To assess the value of CT perfusion imaging in diagnosis of pancreatic adenocarcinoma. Methods: Patients with pancreatic adenocarcinoma (n=39), acute pancreatitis (n=12) and normal pancreas (n=8) were included. Eight seconds after bolus injection of contrast agent (4.0 ml/s, 50 ml [I 300 mg/ml]), dynamic enhancement scanning was carried out for chosen sections by Siemens Bodyperfusion. Scanning parameters were 120-kV tube voltage, 35-mA tube current, 1-second acquisition time, 1-second circle time, and dosage 90.3 CTDIw. After dynamic enhancement scanning, intravenous injection of 50-60 ml contrast agent was given at 2.0 ml/s and routine scanning was performed 25 seconds and 45 seconds after injection. Images were transformed to MV300 workstation and processed with Siemens Perfusion CT/VA11A. Six pseudocolour perfusion images were obtained, and Flow, Flow without Vessels, Blood Volume, Peak Enhancement, Time to Start and Time to Peak were measured. Results: The values of Flow, Blood Volume, Peak Enhancement of pancreatic adenocarcinoma were lower than those of acute pancreatitis and normal pancreas (P<0.01). Time to Start perfusion was not different in the 3 groups; Time to Peak of pancreatic adenocarcinoma group was longer than that of acute pancreatitis group (P<0.05), but had no difference with that of normal pancreas. Pseudocolour perfusion images were convenient for qualitative diagnosis of disease, and perfusion values were needed for quantitative analysis. Pancreatic perfusion of normal pancreas was (0.84±0.32)ml/(min·ml) in this study. Conclusion: Compared with acute pancreatitis and normal pancreas, pancreatic adenocarcinoma has descended Flow, Blood Volume and Peak Enhancement. Pseudocolour perfusion images are beneficial for qualitative diagnosis.
