无醋酸盐和碳酸氢盐血液透析对外周血细胞因子浓度的影响 被引量:3

Effects of acetate free and bicarbonate hemodialysis on serum IL-1,TNFα and sIL-2R

摘要 研究了无醋酸盐血液透析(AFHD)和碳酸氢盐血液透析(BHD)对一些细胞因子和生化参数的影响。8例AFHD和8例BHD患者在实验第1周采用原透析方式,第2周互相交换,所有患者AFHD和BHD前后的结果进行比较显示:AFHD和BHD均能有效地清除尿毒症小分子物质,纠正低钙血症和高磷血症,但AFHD后血磷降低更明显;AFHD和BHD前后外周血肿瘤坏死因子(TNFα)和可溶性白细胞介素-2受体(sIL-2R)浓度均无变化,BHD后白细胞介素-1(IL-1)活性显著增加,而AFHD后IL-1活性也无变化。结果表明AFHD比BHD清除血磷更充分,对影响细胞因子生成的作用更小。 patients who were treated with either bicarbonate or acetate-free hemodialysis thrice aweek, were involved in this two week period study.8 patients were dialysed with bicarbonate dialysate within the first week,and changed to acetate-free hemodialysis(MHD)the next week,others were treated with AFHD followed bicarbonate hemodialysis(BHD).Blood was sampled at the beginning and the endof the first dialysis of the week on each dialvsate. The result indicated that the serum phospho-rous was significantly decreased after both BHD and AFHD,but it was lower after AFHD versus BHD.TNFα and sIL-2R did not changed during BHD and AFHD,Serum IL-1 activity increased markedly dur-ing BHD but unchanged during AFHD. The more effective clearance of phosphorus during AFHD may play a role in the prevention of Osteopathy and cytokines are less affected by AMHD.
