摘要
用高频(10MHz)探头B型超声检查115例不同年龄组正常人与159例不同分期高血压病患者的颈动脉内膜-中层厚度(CAIMT)。观察到正常人CAIMT均值为0.066±0.013cm,正常上限为0.092cm。95%正常人在0.080cm以下。观察者内技术误差仅1.3%、观察者间误差2.7%。但频率较低(5MHz)的探头则严重影响检查准确性。CAIMT与年龄、收缩压、舒张压相关。高血压病患者随着病期增加,CAIMT明显增厚。CAIMT≥0.08cm组患者颈动脉血流Doppler各项参数(颈总动脉收缩期血流速度峰值,舒张末期血流速度,时间平均速度)都比<0.08cm组低,而颈动脉僵硬度、肱动脉内径、左心室重量指数、血纤维蛋白原、甘油三酯比<0.08cm组高。CAIMT与血胆固醇、血高密度脂蛋白胆固醇无关。CAIMT做为一种无创性检查确能判别高血压病患者有无合并动脉硬化。
Abstract Intima-media thickness of the common carotid artery (CAIMT) was investigated in 115 normal subjects and 159 cases of essential hypertension by B mode ultrasonography with Doppler techinque using high frequency (10 MHz) probe, in an attempt to figure out the normal value of CAIMT in adults, and the changes of CAIMT related to the phase of hypertension. It was found that the CAIMT was normally 0. 060±0. 013 cm with upper limit of 0. 092 cm. CAIMT closely related toage,SBP,DBP and increased with the progression of hypertension and the occurrence of carotid atherosclertic plaque. Using CAIMT=0. 080 cm as a cutpoint, in patients with CAIMT≥0.080 cm , the parameters (PS,ED, TAV) of Doppler detected hemodynamics in the carotid artery were lower, while stiffness of the carotid artery, diameter of the brachial artery, left ventricular mass, plasma fibrinogen and triglyceride concentrations were greater than those of patients with CAIMT<0. 080 cm. CAIMT seems not to bear relation to levels of serum cholesterol and HDL-C. All these data suggest that CAIMT could discriminate the existence of atherosclerosis in hypertensive patients.
