摘要
观察140例健康儿童(28天—14岁)及120例先心病患儿的心电图12导联QRS总振幅(∑QRS).健康儿童∑QRS的95%正常值范围为140—250mm.以∑QRS>250mm为诊断儿童左室肥大标准,对先心病并左室肥大诊断的敏感性为83.33%,特异性为97.86%,准确性为91.15%,与其他心电图指标比较,特异性相近,而敏感性及准确性明显提高,认为∑QRS是一项诊断儿童先心病并左室肥大的新指标.
The total 12-lead QRS electrocardiographic voltage as a criterion used for the diagnosis of left ventricular (LV) hypertrophy has been of recent interest. The authors examined the total 12-lead QRS voltage in 140 normal children aged 28 days to fourteen years, and 120 patients with CHD including isolated ASD (11 cases), VSD(45 cases) and PDA (64 cases). Ninety five percent(95%) of the normal values of the total 12-lead QRS voltage in children ranged from 140 to 250mm. With more than 250mm as a criterion for LV hypertrophy this technique yielded a sensitivity of 83.33%, a specificity of 97. 86%, and an accuracy of 91. 15%, the highest ones by any criterion tested. This study showed that ECG total 12-lead QRS voltage more than 250mm is a useful indicator of LV hypertrophy and, in the diagnosis of CHD in children, it is more sensitive than other criteria available.
