摘要
目的对67例单侧Bell麻痹患者同时进行瞬目反射(BR)、神经传导速度(NCV)及神经电图(ENoG)检测,观察三种方法的特异度、敏感度和粗一致性.方法 67例临床确诊的Bell麻痹患者,单侧急性发病;左侧32例,右侧35例,病程2~7 d,进行BR检测,记录同测R1,R2,对侧R2'波,潜伏期及波幅;NCV检测,运动传导诱发电位(M波)潜伏期;ENoG检测,记录ENoG的潜伏期及振幅.结果 BR异常率达100%;NCV异常率达26.9%;ENoG异常率达22.4%.BR与NCV、BR与ENoG的敏感度和粗一致性经χ2检验均有非常显著性差异(P=0.001);NCV与ENoG的敏感度和粗一致性经χ2检验均无显著性差异(P=0.688,P=0.318).结论 BR检测与另两种检测方法比较有非常显著性差异,显示BR检测对判断Bell麻痹神经损伤的敏感性最高,但三种检测方法所反映的面神经损伤情况侧重点略有不同,而用BR检测Bell麻痹的早期诊断的意义最大.
Objective 67 patients with unilateral Bell' s paralysis were checked by Blink reflex ( BR), nerve conduction velocity (NCV) and Electroneurogram (ENoG) meanwhile, to compare these three tests with crude agreement, and observe the sensitivity and specificity of these tests. Methods 67 clinically diagnosed patients with acute unilateral Bell's paralysis were observed, of these cases, lesion in left were 32 cases and in right were 35 cases, and the coures was 2 to 7 days. The incubation period and amplitude of the affected side' s R1 and R2 wave, and those of the other side' s R2 wave recorded underwent BR, the incubation period of motor conduction evoked potential ( M wave) was recorded underwent NCV, and the incubation period and amplitude of EnoG were recorded. Results The abnormality rates of BR, NCV and ENoG were 100% , 26.9% and 22.4% respectively. There was significant difference among BR, NCV and EnoG for sensitivity and crude agreement compared by χ^2 test (P = 0.000), there was on significant difference between NCV and EnoG for sensitivity and crude agreement compared by χ^2 test (P = 0.688, P = 0.318 ) . Conclusions There is significant difference between BR and the other two tests, it shows that BR is the most sensitive method for diahgnosis of Bell' s paralysis. The lesion of facial nerve is reflected differently by these three tests, BR is the most significant test for early diagnosis of Bell' s paralysis.
