摘要
目的:了解首次怀孕妇女睡眠状况,对比有无焦虑、抑郁症状孕妇睡眠质量。方法:①选择2000-05/2004-05在河南大学第一医院和开封妇产医院产科病房住院分娩的产妇75人为孕妇组。均为首次怀孕,且自愿接受各种量表和问卷的调查。按1:1配对的原则从同期本院体检健康人中选择75人为对照组。②评估睡眠质量采用睡眠质量调查表有15个条目,每1个条目下都有3个选项,评分标准为0,1,2分,得分越高表明睡眠质量越差。包括入睡因子、睡眠维持因子、睡眠自评因子、实际睡眠时间和在床上渡过时间(两项之比为睡眠时间之比)、午休时间和梦记忆时间,上述各因子评分相加便得到睡眠质量总分。该量表测评时间为入院后1周内。③抑郁和焦虑状态评估采用抑郁自评量表和焦虑自评量表(各包括20个条目。自评结束后将20个条目各个得分相加,即得到总粗分,然后再经过相关公式换算成标准分。焦虑和抑郁症状自评量表标准分≥50为有焦虑和抑郁症状)。测试时间相同于睡眠质量调查时间。④用t检验进行组间显著性检测。结果:孕妇和健康者各75人均进入结果分析。有焦虑症状孕妇33人,无焦虑症状孕妇42人。有抑郁症状孕妇29人,无抑郁症状孕妇46人。①孕妇组与对照组睡眠质量比较:孕妇组的入睡、睡眠维持、睡眠自评、睡眠总时间因子评分和睡眠质量总分均明显高于对照组(1.09±0.35,0.41±0.04,0.92±0.13,0.96±0.24,4.75±0.97;0.81±0.40,0.29±0.04,0.50±0.08,0.72±0.16,3.92±0.72,t=2.166~3.165,P<0.05~0.01)。②有无焦虑症状孕妇睡眠质量比较:有焦虑症状孕妇的入睡、睡眠维持、睡眠自评、午休时间、睡眠总时间、梦记忆情况因子评分和睡眠质量总分均明显高于无焦虑症状孕妇(1.22±0.40,0.47±0.07,0.92±0.16,1.29±0.43,0.99±0.25,0.74±0.13,4.95±1.05;0.96±0.38,0.36±0.08,0.63±0.15,1.06±0.32,0.78±0.20,0.60±0.06,4.25±0.95,t=2.231~3.127,P<0.05~0.01)。③有抑郁症状孕妇的入睡、睡眠自评、午休时间、睡眠总时间因子评分和睡眠质量总分均明显高于无抑郁症状孕妇(1.14±0.38,0.87±0.15,1.26±0.31,0.95±0.27,4.84±1.01;0.89±0.40,0.69±0.13,1.04±0.52,0.80±0.32,4.19±0.83,t=2.245~2.407,P<0.05)。结论:首次怀孕妇女睡眠质量下降,且有焦虑和抑郁症状的孕妇睡眠质量更差。
AIM: To investigate the sleeping condition of pregnant women for the first time, and compare the sleep quality between pregnant women with and without the symptoms of anxiety and depression.METHODS: Seventy-five first-time pregnant women (pregnant group), who delivered in the sickroom of Department of Obstetrics and Gynecology, the First Affiliated Hospital of Henan University and Kaifeng Hospital for Obstetrics and Gynecology between May 2000 and May 2004, were investigated with the vales and questionnaires voluntarily. Another 75 healthy physical examinees (control group) in the hospital at the same period were selected matched according to the one-to-one ratio. All the subjects were assessed with sleep quality inventory, self-rating depression scale (SDS) and self-rating anxiety scale (SAS) at 1 week after admission. The t test was used for the intergroup comparison.RESULTS: All the 75 pregnant women and 75 healthy people were involved in the analysis of results. There were 33 and 42 pregnant women with and without the symptom of anxiety, 29 and 46 pregnant women with and without the symptom of depression. ① The scores of time to fall sleeping, sleeping maintenance, self-rating sleeping and total sleeping time and the total score of sleep quality were obviously higher in the pregnant group than in the control group (1.09±0.35, 0.41±0.04, 0.92±0.13, 0.96±0.24,4.75±0.97; 0.81±0.40, 0.29±0.04, 0.50±0.08, 0.72±0.16, 3.92±0.72,t=2.166-3.165, P 〈 0.05-0.01).②The scores of time to fall sleeping,sleeping maintenance, self-rating sleeping, time of mid-day rest, total sleeping time and dream memory and the total score of sleep quality were obviously higher in the pregnant women with anxiety than in those without anxiety (1.22±0.40, 0.47±0.07, 0.92±0.16, 1.29±0.43, 0.99±0.25, 0.74±0.13,4.95±1.05; 0.96±0.38, 0.36±0.08, 0.63±0.15, 1.06±0.32, 0.78±0.20, 0.60±0.06,4.25±0.95, t=2.231-3.127, P 〈 0.05-0.01). ③ The scores of time to fall sleeping, self-rating sleeping, total sleeping time and the total score of fsleep quality were obviously higher in the pregnant women with depression than in those without depression (1.14±0.38, 0.87±0.15, 1.26±0.31, 0.95±0.27,4.84±1.01; 0.89±0.40, 0.69±0.13, 1.04±0.52, 0.80±0.32, 4.19±0.83,t=2.245-2.407, P 〈 0.05).CONCLUSION: The sleep quality of the first-time pregnant women is obviously decreased, and the sleep quality is worse in those with the symptoms of anxiety and depression.
