摘要
目的:研究血浆内皮素(ET)、降钙素基因相关肽(CGRP)、一氧化氮(NO)在心脏骤停缺血及再灌注损伤前后含量的变化规律及意义。方法:取家兔40只,随机分为对照组和实验组,制作心脏骤停缺血及再灌注模型,观察心肺复苏期各时相血浆ET、CGRP、NO、血压及心电变化。结果:在心脏骤停及复苏初始血浆ET未见明显升高(P>0.05),心肺复苏5min血浆ET含量升高,血浆CGRP、NO水平降低(P<0.05),复苏后0.5h血浆ET明显升高(P<0.01),复苏后2h血浆ET、CGRP、NO均较复苏前有明显上升(P<0.05)。结论:在心脏骤停缺血及再灌注损伤期血浆ET变化是机体的应激过程和代偿反应,但持续高浓度对机体重要器官将造成进一步损伤,而CGRP、NO有明显拮抗ET异常升高所致的损伤作用,这可能是一种调节机制并在病理生理过程中起重要作用。
Objective: To investigate the alterative regulations and significance of contents of endothelin and calcitonin gene relate d-peptide, nitrogen monoxide in hematoplasma before and after ischemia reperfusion lesion of sudden cardiac arrest of rabbits. Methods: Forty healthy adult rabbits were randomly divided into two groups with 20 rabbits in each group (control and experiment group). The model of ischemia reperfusion lesion of sudden cardiac arrest was built. Blood samples were taken before anesthesia and 0.5,1,2 hour after cardiopulmonary resuscitation for determination of plasma level of ET, CGRP and NO, ECG, HR, BP were monitored during experiment. Results: During sudden cardiac arrest and just after cardiopulmonary resuscitation, ET in plasma had no obvious changes (P〉 0.05). 5 min after resuscitation, ET in plasma was increased, but CGRP and NO were decreased (P 〈 0.05 ), and 0.5 h after resuscitation, ET began to increase (P 〈 0.01 ). Compared with before resuscitation, 2 h after resuscitation, ET, CGRP and NO were obviously increased (P 〈 0.05 ). Conclusion. During ischemia reperfusion lesion of sudden cardiac arrest of rabbits, alteration of ET is a process of stress and a response of compensation, and higher levels of ET in plasma is associated with further injury of major organs. CGRP、NO obviously counteract the effect of further injury that is resulted by increased ET and they can play an important role during pathophysiological duration.
