摘要
目的总结皮肤扩张术在临床应用中存在的问题、主要经验及相关技术,以指导今后工作。方法对20年来临床收治的1454例(1737例次)患者,使用扩张器3620个,进行回顾性分析总结。结果随着经验的积累,手术效果有所提高,总治愈率达96.6%,血肿、感染明显减少。但影响手术效果的问题,如扩张面积不足,皮瓣设计不理想,注水扩张过急过快,张力过大或中央过度扩张等问题仍然存在。结论围绕获取足够"额外"皮肤,如何选择扩张部位、层次,掌握注水扩张规律以及相关技术,总结了经验,提出了建议。
Objective To review the main experiences and problems related in tissue expansion in clinical application, in order to provide guidelines for future work. Methods Reviewed 1 454 patients（1737 cases） which were treated with 3620 tissue expanders in last 20 years. Results With the increase of experience, the results of operation improved. The total cure rate was 96.6%. The complications, such as hematoma formation and infection, decreased significandy. However, there were still problems that affected the results of operation, for example, insufficience of expanded area, imperfect design of skin flap, over-expansion of the expander and flap, and too much tension on the flap, et al. Conclusion In order to get enough “extra” skin, the authors gave some suggestions in choosing the expansion site, the undermine layer, and the techniques of expansion.
