摘要
采用酶联免疫吸附法(ELISA)测定31例慢性肾功能不全(CRI)贫血患者的血清促红细胞生成素(EPO)水平。男22例,女9例,年龄23～68岁,平均42.5±12.9岁。CRI贫血患者血清EPO水平为48.76±35.23ng/L,显著低于正常对照(n=26,107.68±70.68ng/L,t=4.08,P<0.001),且红细胞比积(Hct)与血清肌酐(Scr)之间存在显著负相关(r=-0.7500,F=37.29,P<0.01),而患者Scr与血清EPO水平无线性相关。提示CRI患者血清EPO水平除受肾实质损害影响外,可能还受贫血程度和/或肾脏合成EPO细胞的氧感受器敏感度下降等因素的影响。CRI贫血患者血清EPO与血清铁蛋白、叶酸、维生素B_(12)之间无线性回归关系。
Patients with chronic renal insufficiency (CRD are often associated with anemia. The deficiency of EPO production in the kidney is thought to be a key factor in the pathogenesis of renal anemia. Serum erythropoietin levels were measured by enzyme-linked immunosorbent assay in 26 normal controls and 31 anemic patients (male 22,female 9;age 23-68) with CRI. The primary dis- eases of the patients were chronic glomerular nephritis (28 cases),diabetic nephropathy (3) and lupus nephritis (1). The mean serum EPO level of patients was 48.76±35.32ng/L which was significantly lower than that in normal controls (107. 68±70. 68ng/L,P<0. 001). The results revealed that the Hct values correlated significantly with the serum creatinine (Scr) concentration, but serum EPO levels did not change with an increase in Scr or a decline in Hct values. Moreover,there were no correlations between serum EPO levels and ferritin, folic acid,Vit B12 in patients with CRI. In conclusion,the deficiency of EPO production is the major factor in the pathogenesis of renal anemia. But the damage to the major EPO producing site in the kidney may not be the sole factor for the change of serun EPO levels in anemic patients with CRI,and the (egree of anemia and decreased sensitivity of renal oxygen sensor may be other mechanisms for the changes of serum EPO levels in patients with CRI.
