摘要
采用容量比例概率抽样法(PPS法)对10个省会城市市区各抽取1200名学龄儿童,进行B超法甲状腺体积、尿碘、智力三项指标的调查,对中国城市儿童的碘营养状况做出评估。结果表明,10城市中有3个城市儿童B超法甲状腺肿大率<5％;有5个城市儿童尿碘水平>100μg/L;10城市学龄儿童的智力发育基本上是正常的。根据目前国际推荐的关于“碘缺乏病作为公共卫生问题的判定标准”以及我国制定的“碘缺乏病消除标准”进行分析、评价,10城市中有7城市儿童存在轻度或中度碘缺乏,建议立即采取补碘措施。10城市中北京、上海、武汉虽然儿童甲状腺肿大率<5％,但上海市儿童尿碘水平仍偏低;北京、武汉两市尿碘虽然>100μg/L,但北京市不能排除碘盐影响,而武汉市已供应碘盐达18个月之久。为提高上海市儿童的碘营养水平,维持和巩固北京、武汉两市目前的碘营养现状或已取得的防治效果,建议此3城市仍需采取补碘措施。
In order to assess iodine-nutritional status of school-aged children in cities of China, thyroid volume by ultrasound, urinary iodine and intelligent quotient (IQ) were examined in 12000 school-aged children selected from 10 big cities by using PPS method (1200 from each city). The results showed that in 3 out of 10 cities the goitre rate of children was less than 5% and in 5 cities urinary iodine level of children was greater than 100 μg/L, but the intelligent test showed normal IQ level of children in 10 cities. According to IDD prevalence indicators and proposed criteria for defining iodine deficiency as a public health problem by WHO/UNICEF/IC-CIDD and the National criteria for elimination of IDD,mild or moderate iodine deficiency existed in school-aged children of 7 out of 10 cities and therefore the iodized salt program should be implemented immediately. In other 3 cities,Beijing,Shanghai and Wuhan,although the goitre rate of children was less than 5% ,the urine iodine level was still lower(<100μg/L)in Shanghai city, and iodized salt has been already used by some families in Beijing city and iodized salt program has been implemented in Wuhan for 18 months. So iodine supplement program should be implemented in these 3 cities.
