碘伏冲洗对全膝置换术后切口及功能恢复的影响 被引量:7

摘要 目的探讨不同的手术冲洗液对全膝置换术后患者切口及功能恢复的影响。方法 150例全膝置换患者随机分为生理盐水组(50例)、庆大霉素液组(50例)及碘伏组(50例),术中分别使用相对应的手术冲洗液,通过观察切口的愈合情况和术后2周及术后1年膝关节功能的HSS评分,比较3种冲洗液的效果。结果庆大霉素液组的切口愈合优良率(90%)明显优于生理盐水组(84%)及碘伏组(76%),庆大霉素液及碘伏冲洗切口在预防感染方面并不优于生理盐水组(P>0.05)。术后2周碘伏组膝关节功能恢复明显差于生理盐水组(P<0.05)及庆大霉素液组(P<0.05)。随访1年时3种冲洗液在患者膝关节功能的恢复方面无差异(P>0.05)。结论术中使用碘伏原液冲洗切口影响患者手术切口的愈合和近期膝关节功能的恢复。 Objective To explore the effects of different operation fluids on incision healing and functional recovery after total knee arthroplasty.Methods 150 cases of total knee arthroplasty were randomly divided into normal saline group(50 cases),gentamicin solution group(50 cases) and iodine group(50 cases),and during the operation used,the solutions were the corresponding fluid.The effects of 3 kinds of fluids were compared by observing the healing of incision and the HSS score of knee joint function after 2 weeks and 1 year.Results The excellent and good rate of incision healing of gentamicin solution group(90%) was significantly better than that of normal saline group(84%) and iodine group(76%).The prevention of infection of gentamicin solution group and iodine group was not better than that of normal saline group(P>0.05).The recovery of keen joint function of iodine group was significantly worse than that of normal saline group(P<0.05),and gentamicin solution group(P<0.05) after 2 weeks postoperation.There was no significant difference in function recovery during 3 groups after 1 year follow-up(P>0.05).Conclusions The original iodine washed incision during operation can affect the healing of incision and the short-term recovery of keen joint function.
