摘要
目的观察小鼠内毒素血症初期凝血功能的变化及脏器损伤情况,探讨早期应用肝素的保护作用。方法观察Balb/c小鼠对照组(A组)、内毒素(B组)及内毒素加肝素(C组)肛温、PLT、WBC、纤溶酶原(PLG)及抗凝血酶-Ⅲ(AT-Ⅲ)活性等指标,并取肺、肝、脾观察病理学变化。结果1.A、B、C 3组中任何2组组间PLT比较差异均无显著性(P>0.05),B、C组在处理后24、h与相应时间点对照组比较,肛温、WBC、PLG及AT-Ⅲ减低,有显著性差异(P<0.05);2.C组PLG比相应时间点B组高,差异有显著性(P<0.05)。C1组AT-Ⅲ活性高于B1组,有显著性差异(P<0.05);3.病理结果:B组小鼠肺、肝、脾表面有明显出血征象,镜下炎症细胞浸润渐加重;C组相应时间点与B组比较病理改变轻微。结论内毒素血症初期即有凝血及纤溶系统激活,脏器损害;早期应用肝素对DIC前期和脏器损害具有治疗和保护作用。
Objective To observe the changes of coagulation mechanism in the early- phase of infections and the pathological changes in lipopolysaccharides(LPS) mice, and to explore the protective role of early application of heparin. Methods Male Balb/c mice were injected LPS or LPS with heparin, the rectal temperature, white blood cell(WBC)and platelet(PLT), plasmhlogen (PLG) and antithrombin- Ⅲ (AT-Ⅲ )activity were detemlined for all groups ; after the mice were killed, lungs, livers and spleens of mie were removed for pathological observation. Results 1. Tbere was no significant difference in PLT among the 3 groups ( P 〈 0.05 ). The rectal lemperature, WBC, PLG and AT -Ⅲ in group B were significantly lower ( P 〈 0.05 ) than those in the control group at the same point of time. 2. In group C, PLG was significantly higher than group B at the ,same point of time (P〈0.05). In group C1, AT- Ⅲ was significantly higher than group B1 (P〈0.05). 3. The pathological changes: spotted bleeding could be seen on the surface of the lungs, the liver and the spleen in group B, and under the electron microscope many inflanmmtory cells infiltration in group B ap peared. The pathological changes of group C were slighter than those in group B at the same point of time. Conclusions In the early phase of indotoxemia, there is the consumption of coagulation and fibrinolysis, and organ danmges appears. The early application of heparin may play a positive role in the treatment and protection of Pre - DIC.
