猪全层皮肤缺损创面新生上皮下细胞增殖与凋亡的动态变化

摘要 目的探讨猪皮肤缺损创面新生上皮下细胞增殖与凋亡的动态变化.方法在6只小型猪的背部各造成6个直径为4cm的圆形全层皮肤缺损创面.在创面形成后当日及6、12、15、20、25、40天,取创面及创面边缘的组织,采用TUNEL法检测细胞凋亡情况,采用Ki67免疫组化染色观察创面内细胞增殖情况.结果平均创面愈合时间为(24.5±2.1)天.正常真皮内检测不到细胞凋亡;伤后第12天,创面边缘新生上皮下的凋亡指数(凋亡细胞数/400×视野)增高至3.11±0.266,伤后第25天开始下降,为1.88±0.337;创面中心的凋亡指数在伤后第20天才明显增高,为2.30±0.258;伤后第40天(创面愈合后15天),新生上皮下检测不到细胞凋亡;伤后12~20天,创面边缘新生上皮下的凋亡指数和创面中心的凋亡指数有明显统计学意义(P<0.01).伤后第6天开始,创面中心增殖指数(增殖细胞数/400×视野)增高至142.7±14.18,此后逐渐下降;创面边缘的增殖指数在伤后第6天轻度增高,此后维持在较低水平;伤后6~20天创面中心增殖指数明显高于创面边缘(P<0.01).结论在猪全层皮肤缺损创面愈合过程中,肉芽组织在被新生上皮覆盖后不久,凋亡指数增高,增殖指数降低;猪全层皮肤缺损创面新生上皮下细胞成分的减少可能与凋亡指数增高和增殖指数下降有关. Objective To investigate the mechanism of the regression in cellularity under neoepithelium in minitype pigs'full thickness dermal wounds. Methods After the establishment of full-thickness dermal wounds with the diameter of 4 em on the back of 6 minitype pigs,the specimens were eollected from wound edge and wound eenter immediately after injury ,6,12,15,20, 25 and 40 days later. The specimens were subjected to TUNEL technique to detect apoptosis,tu immunohistochemistry of Ki67 to detect proliferative cells. Results The mean healing time was 24.5 ± 2. 1 days( x ± s ). Apoptusis was not deteeted in normal dermis in this experiment ; apoptotie index ( the number of apuptotic cells per 400 power field ) of the tissue under neoepithelium increased to 3. 11 ± 0. 266 12 days after injury and decreased till 25 days after injury;that in wound center was less than 0.3 from 6 to 15 days after injury and increased to 2.30 ±0. 258 20 days after injury;from 12 to 20 days after injury,there were significant statistical difference between the apoptotie index of the tissue under neoepithelium and that in wound center( P 〈 0.01 ). Proliferative index ( the number of proliferative cells per 400 power field ) in wound center increased to 142.7 + 14. 18 on day 6 after injury, then decreased gradually and was 19.2 ± 11.46 on day 20 ; that under neoepithelium peaked at 14.5 ± 5.25 on day 12 after injury ; from 6 to 20 days after injury, there were significant difference between the proliferative index in wound center and that under neocpithelium ( P 〈 0.01 ). Conclusion Shortly after covered by neocpithelium,the Apoptotie Index of granulation tissue increased and the Proliferative Index decreased;regression in cellularity of the tissue under neoepithelium might be due to the increase of Apoptotic Index and the decrease of Proliferative Index.
