摘要 目的观察α甲酰辅酶A消旋酶(AMACR)和细胞角蛋白7(CK7)在乳头状肾细胞癌(PRCC)中的表达,探讨其诊断价值。方法收集PRCC病例23例,另用26例肾透明细胞癌作为对照。AMACR和CK7免疫组化检测采用Envision二步法。结果23例PRCC中20例(87%)肿瘤组织AMACR阳性,其中强阳性15例;26例透明细胞癌中10例AMACR阳性(38%),其中仅1例强阳性(P<0.005)。23例PRCC中16例(70%)肿瘤组织CK7阳性,其中强阳性14例;26例透明细胞癌仅2例(8%)弱阳性(P<0.001)。11例Ⅰ型PRCCCK7均阳性(100%),12例Ⅱ型PRCC中5例阳性(42%)(P=0.003)。结论检测AMACR和CK7表达有助于鉴别PRCC和肾透明细胞癌;CK7表达在二型PRCC间差别显著。 Objective To observe the expression of alpha-methylacyl-CoA racemase (AMACR) and cell keratin 7 (CK7) in papillary renal cell carcinoma (PRCC) and investigate the significance in pathological diagnosis of renal cell carcinoma. Methods Pathological materials of 23 patients with PRCC were collected and 26 patients with renal clear cell carcinoma were Results In 23 PRCC patients tive. While 10 of 26 (38%) used as controls. Envision immunohistochemistry of AMACR and CK7 was applied. , 20 (87%) were found AMACR positive in tumor cells with 15 cases strongly posipatients with clear cell carcinoma showed AMACR positive with only 1 case strongly positive (P 〈0. 005). Sixteen of 23 (70%) PRCC patients were CK7 positive with 14 cases strongly positive. Only 2 (8%) cases of clear cell carcinoma showed weakly positive for CK7 (P 〈0. 001 ). All 11 patients with type 1 PRCC were CK7 positive and 5 of 12 (42%) type 2 PRCC patients were positive (P = 0. 003). Conclusion Analysis of AMACR and CK7 expression is helpful in differentiating PRCC and renal clear cell carcinoma, and CK7 expression is significantly different in two types of PRCC.
