摘要
目的探索自主神经活动在阵发性心房颤动中的作用。方法在动态心电图上测量分析了25例阵发性房颤患者53阵次心房颤动发作前后及白天穴6:00~22:00雪和夜间穴22:00~6:00雪的心率变异指标穴SDNN、LF、HF、LF/HF雪的变化。结果房颤发作前1hSDNN、HF较终止后1h显著升高穴p<0.01雪,LF/HF显著降低穴p<0.01雪,LF无变化;LF、HF白天均高于夜间穴分别为p<0.05和p<0.01雪,夜晚LF高于HF穴p<0.05雪,白天与夜间SDNN、LF/HF无差异,白天LF与HF无差异。结论阵发性心房颤动患者存在着明显的自主神经功能失常,特别是白天迷走神经张力增强;迷走神经张力增强是阵发性房颤的重要原因。
Objective To explore the role of autonomic nervous dysfunction before and after paroxysmal atrial fibrillation. Methods SDNN,LF,HF,LF/HF of heart rate variability were measured with 24 hour dynamic ECG recording in 25 patients with paroxysmal atrial fibrillation. Results SDNN, HF before atrial fibrillation onset increased significantly than that after stopping of paroxysmal atrial fibrillations (p〈0.01),LF/HF decreased remarkably (p〈0.01). LF and HF in day increased significantly than that in night (separately,p〈0.05,p〈0.01),and LF was more high than HF in night (p〈0.01). Condusions The results suggest that there were dysfunction of autonomic nerve in the patients with paroxysmal atrial fibrillation,especially,the vagal activation increase in day. And the attenued vagal activation causes beginning of paroxysmal atrial fibrillation.
