血清转化生长因子β_1及肝细胞生长因子与慢性乙型肝炎早期肾损害关系的研究

摘要 目的探讨血清转化生长因子β1(TGF-β1)、肝细胞生长因子(HGF)与慢性乙型肝炎(CHB)早期肾损伤的关系。方法对2004年泸州医学院附属医院感染科44例慢性乙型肝炎(轻度)住院患者用双抗体夹心ELISA法和放射免疫法测定血清TGF-β1、HGF和尿α1微球蛋白(α1-MG)、尿微量白蛋白(mALB)质量浓度并与20名健康人作对照。结果44例CHB中早期肾损害组24例,无肾损害组20例;慢性乙型肝炎患者血清TGF-β1、HGF高于对照组(P<0.01);早期肾损害组患者血清TGF-β1、HGF明显高于无肾损害组患者(P<0.01);且血清TGF-β1、HGF与早期肾损害指标:尿α1-MG、mALB质量浓度呈正相关(r分别=0.718,0.683,0.664,0.637)。结论慢性乙型肝炎早期肾损害患者血清TGF-β1、HGF均升高,可能是机体损害与抗损害反馈调节所致。 Objective To investigate the role of transforming growth factor - β1 ( TGF - β1 ) and hepatocyte growth factor (HGF) in the pathogenesis of early renal impairment in patients with chronic hepatitis B (CHB). Methods The serum levels of TGF - β1 and HGF of 44 patients with CHB and 20 healthy people were measured by specific - ELISA, and the urine levels of α1 -microglobulin (α1 - MG) and microalbumin (mALB) were tested by RIA. Results The serum levels of TGF -β1 and HGF in the patients with CHB were significantly higher than those in the control ( P 〈 0. 01 ). The serum levels of TGF - β1 and HGF in the patients with early renal impairment were higher than those in patients without early renal impairment (P 〈0. 01 ) , and positively correlated with the urine α1-MG and mALB levels (r = 0. 718, 0. 683, 0. 664, 0. 637, respectively). Conclusion It may be a feedback mechanism that the levels of HGF increase to antagonize the hurt by the TGF - β1 in the early renal impairment in patients with CHB.
