摘要
目的探讨乌头类生物碱在体外循环心内直视手术中抗心肌缺血再灌注损伤作用及其机制。方法将患者随机分成对照组和乌头类生物碱组。于开放主动脉时、体外循环结束时、手术结束时、术后1d等各个时间点测两组患者血清肌酸激酶同工酶(CKMB)、肌钙蛋白(cTnI)、超氧化物歧化酶(SOD)、丙二醛(MDA)含量;于停机时测心肌组织SOD、MDA含量。结果乌头类生物碱组CKMB水平分别为21.63±4.54,29.73±6.53,37.78±6.55,28.74±7.16,与对照组比较差异有统计学意义(P<0.05);cTnI值分别为0.337±0.141,0.497±0.271,0.758±0.399,0.542±0.244,与对照组比较差异有统计学意义(P<0.05);SOD值分别为75.56±20.09,77.01±20.36,91.39±19.56,108.44±24.23,与对照组比较差异有统计学意义(P<0.05);MDA水平分别为4.14±1.88,7.48±2.97,7.86±3.44,6.79±3.21,6.16±3.30,与对照组比较差异有统计学意义(P<0.05)。术后乌头类生物碱组心肌组织MDA值明显低于对照组(P<0.05),而SOD值则明显高于对照组(P<0.05)。结论乌头类生物碱对体外循环心肌缺血再灌注损伤具有保护作用。
Objective To discuss the efficacy and mechanism of anti-myocardial ischemia reperfusion injury of aconitine akaloid (Sini decoction) (SND) in extracorporeal circulation (ECC), Methods The cardiac valve replacement patients were divided into control group and SND group. Serum CK-MB, cTnI, SOD and MDA were detected at different time points including opening aorta, termination of ECC, end of operation, one day after operation and so on, Myocardial SOD and MDA were measured before-operation and at the time of ECC termination, Results The serum levels of CK-MB in SND group were 21.63 ± 4.54, 29.73 ± 6.53, 37.78 ± 6.55 and 28.74 ± 7.16 respectively, significantly lower than those in control group (P 〈 0.05). The levels of cTnI in SND group were 0. 337 ± 0. 141, 0. 497 ± 0. 271, 0. 758 ± 0. 399 and 0. 542 ± 0.244 respectively, significantely lower than those in control group ( P 〈 0.05) .The serum levels of SOD in SND group were 75.56 ± 20.09,77.01 ± 20.36,91.39 ± 19.56 and 108.44±24.23, significantely higher than those in control group (P〈 0.05). The levels of MDA in SND group were 4. 14 ± 1.88, 7.48 ± 2.97, 7.86 ± 3.44, 6.79 ± 3.21 and 6.16 ± 3.30 significantly, significantly lower than those in control group (P 〈 0.05 ), The level of myocardium MDA post-operation in SND group was significantly decreased as compared with control group ( P 〈 0.05), The activities of myocardial SOD in SND group were significantely increased as compared with control group (P 〈 0.05 ), Conclusion SND can protect effectively myocardial ischemia reperfusion injury in ECC of cardiac valve reolacement.
