摘要
报告102例下肢多发骨折患者,骨折共258处。56.8%合并颅脑、胸、腹多发损伤。经救治无急性死亡。下肢多发骨折处理以股骨骨折为主,合并同侧髋、膝关节损伤及脊柱骨盆损伤者,强调内固定处理。下肢功能恢复良好率达85.4%。
102 cases of multiple fractures of lower extremity in the multiple trauma patient were successfully treated in rccent years There were 258 fracturcs, 56.8% of these patients were complicated by cranial, chest or abdomcnal trauma. 34.3% complicated by shock,ARDS or acute renal failure. The principles of management are that injuries of cranial, chest or abdomenal vescera, and open wound of large vessel injury are treated first, the fractures immobilized by traction or splint temporally and treated in second stage. early discovery and management of complication All the vesceral injury and severe complications healed smoothly. No death occurred in acute stage but one died due to mediastinum infection. Interal fixation of the femoral fracture is the key to successful management of multiple fracture The fractures of ipsilateral hip or knee joint should be fixed by internal fixation. For this purpose multiple pin fixation and modified tension band wire were set up. Early mobilization of joints were carried out. Excellent and good results were obtained in 84.9% of the cases treated by this method
