酶法与苦味酸法测定血清肌酐水平的转换方法 被引量:6

摘要 目的:探索酶法与苦味酸法测定血清肌酐在不同水平段的测定偏差,并建立新旧方法测定结果的转换方法,以满足临床医生的需求。方法:用两种血清肌酐测定方法同时测定667例来我院门诊就医的不同疾病患者血清肌酐水平,并根据酶法测定值,将其分为5个水平组,统计两种方法的测定数据。结果:酶法与苦味酸法测定值在各水平组的平均值分别为:A:64.0μmol/L,92.7μmol/L;B:182.5μmol/L,201.9μmol/L;C:378.4μmol/L,384.6μmol/L;D:561.2μmol/L,549.4μmol/L;E:1077.7μmol/L,1025.7μmol/L。两种方法相关均良好,相关系数分别为0.971,0.987,0.950,0.983,0.994。结论:血清肌酐水平较低时,酶法测定值较与苦味酸法测定值低;在血清肌酐水平为471μmol/L以上时,酶法因方法学优于苦味酸法,而使测定值能够达到真实水平;若已知酶法测定值,临床医生可通过以下公式y(苦味酸法)=0.9186x(酶法)+34.385,得出苦味酸法测定值。反之,则可通过公式y(酶法)=1.0872x(苦味酸法)-37.065,得出酶法测定值。 Objective: To evaluate the discrepancy between Creatinine's enzymatic method and picric kinetic method. Try to found the conversion equation to transfer the results between two methods. Methods:Measure creatinine's results of 667 patients ' serum with two methods in parallel, then according to enzymatic method, divide the results into 5 level-groups. Averages, standard variances were compared. Results: It showed that all groups had good correlations with two methods (A:0. 971, B:0. 987, C :0. 950, D :0. 983, E: 0. 994 ). Averages of 5 level-groups were calculated ( A: 64. 0μmol/L, 92.7μmol/L; B : 182. 5 μmol/L, 201.9μmol/L; C: 378.4μmol/L, 384. 6μmol/L; D: 561. 2μmol/L, 549. 4μmol/L; E: 1077. 7μmol/L, 1025. 7μmol/L). Conclusion:Creatinine' s results with two methods could be switched by equations y( picric method) = 0.9186x( enzymatic method) + 34. 385 and y( enzymatic method) = 1. 0872x( picric method) - 37. 065.
