摘要
脓毒症、多器官功能障碍综合征发病机制及其防治是目前创、烧伤临床亟待解决的重大课题,开展其研究具有极其重要的理论意义和实用价值。脓毒症的主要研究方向包括:①金黄色葡萄球菌外毒素及其致病作用;②模式识别受体与信号转导机制;③基因多态性研究;④“晚期炎性介质”———高迁移率族蛋白-1;⑤基因组与蛋白组学;⑥脓毒症相关定义新认识和分阶段诊断系统;⑦防治新措施与循证医学等。深入探索和认识脓毒症的确切发病机制和干预措施,必将导致脓毒症防治策略上的进步和革命。
The present article dealt with the pathogenesis and treatment of sepsis/multiple organ dysfunction syndrome (MODS), which is currently recognized as a problem of utmost importance in the field of critical care medicine, especially in trauma and bum surgery. It is of great significance to further elucidate the pathogenetie mechanisms, and to seek novel intervention strategies to prevent and treat sepsis/MODS secondary to severe trauma or bums. The main aspects of sepsis research are briefly summarized as follows: (1) Gram-positive organisms as well as Staphylococcal exotoxin in particular and sepsis; (2) pattern recognition receptors and signal transduetion; (3) genomie polymorphisms in sepsis and their association with susceptibility; (4) novel delayed inflammatory eytokine: high mobility group box-1 protein; (5) genome, transefiptome, and proteome in sepsis; (6) new diagnostic criteria for sepsis and the PIRO system for staging sepsis; and (7) novel therapeutic strategies for sepsis and evidence-based recommendations. As we gain a more complete understanding of the pathogenetic mechanisms of sepsis a large number of potential therapeutic intervention targets will be disclosed. Hopefully, in the near future new therapies will be successfully exploited to arrest the pathophysiologic alterations seen in sepsis following major trauma or bums.
