摘要
目的研究血液灌流对严重酒精患者血流动力学和血中乙醇浓度的影响.方法严重酒精中毒患者7例,在呼吸机控制呼吸情况下右侧颈内静脉置入Swan-Ganz导管于血液灌流前及血液灌流后30、60及120min监测血流动力学的变化,用气相色谱法于血液灌流前、后测定血中乙醇的浓度.结果7例患者在血液灌流后1 h内清醒者3例,1-3 h内清醒者4例,均于血液灌流后10 h内撤离呼吸机.血液灌流前SVRI显著降低,血液灌流后120min内MAP、SVRI、PVRI升高非常显著,HR降至正常范围.血液灌流2 h后血中乙醇的浓度由(0.59±0.08)%降至(0.03±0.03)%.结论血液灌流和呼吸机支持治疗及补充血容量等对严重酒精中毒的症状、血流动力学有明显的改善作用.血液灌流能有效地降低血液中的乙醇浓度.
Objective To study the effects of hemoperfusion on hemodynamics and serum concentration in the patients with severe alcohol intoxication.Methods 7 patients with severe alcohol intoxication were involved and hypotension, respiratory failure and deep coma were the main clinical manifestation. The hemodynamics were measured before hemoperfusion, 30, 60 and 120 min after hemoperfusion through Swan - Ganz catheter. The serum concentration of alcohOl was analyzed before and after hemoperfusion by the use of gas chromatography. Results 7 patients recovered after hemoperfusion and respiratory support. MAP, SVRI and PVRI increased significantly 120 min after hemoperfusion compared with before hemoperfusion and HR returned to normal rangSs within 120min after hemoperfusion . The serum concentration of alcohol decreased from (0.59±0.08) % to (0.03±0.03) % 2 h after hemoperfusion. Conclusion Hemoperfusion with respiratory support and infusion has a beneficial effects on hemedynamics and clinical manifestation in the patients with severe alcohol intoxication and can eliminate alcohol in the blood effectively.
