摘要
目的探讨以桡动脉(radial artery,RA)为主的动脉材料在全动脉化冠状动脉旁路移植术(coronary artery by-passgrafting,CABG)中的疗效.方法从2000年2月至2005年4月,进行72例全动脉化CABG.每例均采集RA与左内乳动脉(1eft internal mammary artery,LIMA)进行CABG.术后密切随访.结果平均每例植入动脉桥3.7±0.91支,其中LI-MA72支,RA桥124支.RA单纯端侧吻合45支,序贯吻合79支.Y型桥32支.全组病例仅1例(1.39%)死于室壁瘤切除后发生低心排并急性肾功能衰竭,另有3例术后出现急性肾功能衰竭,2例出现脑血管意外,3~4周后恢复出院.无一例出现术后围手术期心肌缺血.术后平均随访31±1.4月(6~51月),无一例出现心绞痛或心肌缺血.结论以RA为主的全动脉化CABG安全而且疗效好,RA有望在CABG中发挥重要的作用.
Objective To investigate the effect of arterial grafts mainly involving radial artery (RA) on total arterial revascularization (TAR) in coronary artery bypass grafting (CABG). Methods From February 2000 to May 2005, 72 patients were treated with TAR in coronary artery bypass grafting. RA and left internal mammary artery (LIMA) were used in each CABG and follow-up were conducted. Results Arterial material of averaged 3.7±0.91 vessel including 72 LIMA and 124 RA were employed in CABG. 45 single anastomosis of RA and 79 sequential ones including 32 Y grafts were performed. Only one patients (1.39%) died of low output syndrome and acute renal function failure in CABG and left ventricular aneurysmectomy. 3 acute renal function failure and 2 cerebral accident occurred, with total rehabilitations after the therapy of 3-4 weeks. No angi- na pectoris and infarcts happened in averaged follow-up of 31±1.4 months. Conclusion Using arterial grafts mainly involving RA, TAR in CABG is safe, good results can be achieved and RA may be play an important role in TAR.
