摘要
观察两种钢丝钩状电极在紧急心脏起搏中的临床应用情况,初步总结多孔表面钢丝钩状电极的特点。32例住院期间发生心脏骤停的患者,在心肺复苏时,随机使用普通钢丝钩状电极和多孔表面钢丝钩状电极进行紧急心脏起搏治疗。依所用电极种类,归入相应组。比较两组建立有效起搏的时间、起搏参数和起搏并发症。结果:两组紧急起搏的成功率均为100%,建立有效起搏操作时间无显著性差异,均未发生心包压塞等严重并发症。多孔表面细钢丝钩状电极与普通钢丝钩状电极比较,起搏阈值降低25%,R波振幅升高38.1%,R波斜率升高13.3%,电极阻抗降低12.5%。结论:多孔表面钢丝钩状电极操作性能良好,能显著降低起搏阈值,提高对心室电活动的感知能力,有助于建立迅速、稳定、有效的紧急心脏起搏,达到抢救心脏骤停的目的。
The clinical features of two types of steel wire electrode had been observed in emergent cardiac pacing, and the characteristic of fractal steel wire electrode was primarily concluded. Ordinary and fractal steel wire electrode were randomly used in the process of cardiac resuscitation in thirty-two cardiac arrest patients. According to electrode utility, patients were distributed in two groups. Results : The operation success rate were 100 percent without severe complications such as cardiac tampond in all patients. Compared with ordinary steel wire electrode, fractal steel wire electrode showed a decrement of pacing threshold and electrode impedence, and an increment of amplitude and slope of QRS. Conclusion: Fractal steel wire electrode shows good steerablity and effectiveness in emergent cardiac pacing for cardiac arrest.
