云南省主要感染季节不同阶段人群血吸虫感染情况观察

摘要 目的了解云南省血吸虫病重疫区主要感染季节的不同阶段居民血吸虫感染、再感染情况,为制定更具针对性的防治措施提供依据. 方法根据农事活动和季节变化将主要感染季节分为前期、中期和后期3个阶段.选择高原峡谷型血吸虫病流行区洱源县的4个自然村作为观察点,将6~60岁居民随机分为Ⅰ、Ⅱ两组,Ⅰ组人群观察主要感染季节前期和后期血吸虫感染、再感染情况;Ⅱ组人群观察主要感染季节中期血吸虫感染、再感染情况.在各阶段开始前10 d用吡喹酮治疗,观察期结束后30 d查病.同时选择30名学生作为记录员,记录各自家庭成员每天接触疫水的时间. 结果在主要感染季节前期、中期和后期居民血吸虫感染率分别为32.17%、12.42%、7.10%,平均每人每天接触疫水时间分别为(82±26) min、(30±14) min和(14±6) min. 结论云南省重疫区主要感染季节的前期(栽插期)血吸虫感染率最高,主要与感染性钉螺密度高和居民接触疫水时间长有关. Objective To investigate the status of schistosome infection and reinfection in different stages in main infection season among residents in heavy epidemic areas in Yunnan Province and to provide evidence for making more effective measures of schistoso- miasis control. Methods According to the arrangement of farm work and the changes of the season, the main infection season was divided into 3 stages: early stage(20 May-20 June), middle stage(21 June-10 August) and late stage(11 August-10 October). Four villages located in the plateau basin in Eryuan County were selected as observatory spots. The local residents aged 6-60 were randomly divided into Group Ⅰ and Group Ⅱ. Residents in Group Ⅰ were observed to obtain the data of schistosome infection and reinfection in early and late stages, while Group Ⅱ were observed in middle stage. In the first 10 days of each stage, the selected people were treated with Praziquantel; and examined after 30 days of each stage. Meanwhile, 30 students were chosen as recorders to record their family members' time length of contacting infected water each day. Results Residents' schistosome infection rates in early, middle, late stage were 32.17%, 12.42%, 7.10% respectively. Each person's average time length of contacting infected wa ter per day in three stages were (80±26) min, (30±14) min and (14±6) min respectively. Conclusion Schistosome infection rate in heavy epidemic areas among residents in early stage is the highest in Yunnan, which is related to the density of infected snails and the length of time residents contact the infected water.
