Effect of hyperbaric oxygenation on traumatic intractable brain edema

摘要 目的探讨高压氧治疗外伤性顽固性脑水肿的疗效。方法将100例该病患者随机分成A、B两组。A组53例,在常规治疗情况下加用高压氧治疗(0.2MPa,吸纯氧40min×2次,其间吸空气10min)。B组47例,为常规治疗组,在使用甘露醇、速尿的基础上加用甘油及七叶皂苷等药物治疗。结果高压氧治疗外伤性顽固性脑水肿疗效明显优于常规药物治疗。结论对临床众多的外伤性顽固性脑水肿患者来说,高压氧治疗是目前疗效最好的方法。 Objective To study the effects of hyperbaric oxygenation (HBO) on traumatic intractable brain edema. Methods 100 cases with traumatic intractable brain edema were randomly divided into HBO group (53 cases) and control group (47 cases). HBO group was given the comprehensive treatment composed of normal treatment with mannite and/or furosemide, glycerine and saponin and HBO by exposure to 100% hyperbaric oxygen chamber with 0.2 Mpa for 40 minutes and to air for 10 minutes reciprocally. The controls were given normal treatment only. One course lasted 10 days in both groups. Based on CT examinations, the outcome of treatment was classified into 4 categories: complete absorption, improvement, less improvement, and no changes, the first two considered as effective and the last two as ineffective. Results HBO was more effective than normal therapy by rank-sum test (P〈0.0001). Conclusion HBO is the best among the optional therapies to traumatic intractable brain edema.
