摘要 目的探讨高、中、低3种功率的Nd∶YAG激光脑间质内热疗致脑水肿的差异,为临床应用提供实验依据。方法成年新西兰大白兔15只,随机分为2.5W×240s、7W×20s、30W×3s3组,对兔两侧大脑半球进行Nd∶YAG激光脑间质内热疗实验,3d后开颅取脑,行肉眼和镜下观察,测定凝固性坏死和脑水肿的范围。结果激光脑间质内热疗的毁损灶呈球形或类球形,2.5W×240s组及7W×20s组较30W×3s组病灶形状更规则;2.5W×240s组脑水肿程度较轻于7W×20s组及30W×3s组(P=0.026,P=0.043)。结论为减少脑水肿,在进行激光脑间质内热疗时,应以不产生气泡或热膨胀压强为宜,即应选择小功率激光进行照射。 Objective :To explore brain edema due to hyperthermia of brain interstitial tissue by Nd : YAG laser and its difference after irradiation of 3 levels of laser power in order to provide experimental evidences for clinical application. Methods :Fifteen adult rabbits were divided into three groups and the experiment was performed on bilateral hemispheres. The parameter of laser of group A was 2.5 W ×240 s, group B was 7 W × 20 s and group C was 30 W × 3 s. After hyperthermia of interstitial tissue, we observed the common condition and removed the brain carefully and fixed in Formalin after sacrificing them three days later. Histological changes were observed through microscope and the scope of coagulative necrosis and edema was measured. The brain ede- ma was compared by analysis of t test, Results Through the experiment, we found that Nd : YAG laser-induced hyperthermia of interstitial tissue could form a fine structure of zonal lesion. The brain edema of group A was lighter than that of groups B and C ( P =0. 026 ,P =0. 043) Conclusions In order to reduce brain edema, the output of laser should be enough low, and does not produce bubble and thermal expanding pressure.
